Job summary:As a Licensed Practical Nurse (LPN), you will play a vital role in delivering quality nursing care to patients under the supervision of registered nurses (RNs) and other healthcare professionals. Key Responsibilities:
- Patient Care and Assistance:
- Provide direct patient care, including monitoring vital signs, administering medications, dressing wounds, and assisting with daily living (ADLs) such as bathing, feeding, and toileting.
- Collaborate with registered nurses (RNs) and other healthcare team members to implement care plans and ensure continuity of care.
- Assess patients' physical and mental status, report any changes or concerns to the supervising RN or healthcare provider, and document observations accurately and timely.
- Medication Administration and Documentation:
- Administer medications orally, intramuscularly, subcutaneously, and intradermally as prescribed by healthcare providers, following established protocols and medication administration procedures.
- Document medication administration, patient responses, and any adverse reactions or side effects in electronic health records (EHRs) or other designated documentation systems.
- Monitor patients for signs of medication effectiveness, therapeutic responses, and adverse reactions, reporting findings to the supervising RN or healthcare provider.
- Patient Education and Support:
- Educate patients and their families about their health conditions, treatment plans, medication instructions, and self-care techniques.
- Provide emotional support, empathy, and encouragement to patients and families, addressing their concerns, questions, and informational needs.
- Foster therapeutic relationships with patients, promoting trust, dignity, and respect in all interactions.
- Collaboration and Communication:
- Communicate effectively with healthcare team members, including RNs, physicians, therapists, and other ancillary staff, to coordinate patient care, exchange information, and ensure timely interventions.
- Participate in interdisciplinary team meetings, care conferences, and patient rounds, contributing insights and observations to enhance care planning and decision-making.
- Maintain open lines of communication with patients and families, keeping them informed about care plans, treatment options, and changes in their condition.
- Quality Improvement and Professional Development:
- Participate in quality improvement initiatives, infection control measures, and safety protocols to promote optimal patient outcomes and prevent healthcare-associated infections.
- Pursue opportunities for continuing education, training, and professional development to enhance clinical skills, expand knowledge, and stay current with best practices and regulatory requirements.
- Adhere to ethical and legal standards of nursing practice, maintain confidentiality, respect patient rights, and uphold professional integrity.
- Active licensure as a Licensed Practical Nurse (LPN) in practice.
- Basic Life Support (BLS) certification required; additional certifications may be required based on employer preferences or specialty areas.
