The Hospital Liaison (will participate in the hospital discharge planning process, working collaboratively with the Case Management Department at the following Berkshire Health System (BHS) facilities; Berkshire Medical Center (BMC), Fairview Hospital (Fairview), and North Adams Regional Hospital (NARH) to promote skilled nursing facility diversions. The Hospital Liaison will assist case management staff in connecting individuals to home and community-based services and will spend time each week at each facility. Graduation from an accredited college or university with a Bachelor’s Degree in Human Services or a related field and two years relevant experience or completed an approved practical nurse diploma or certificate program and have a current and valid Massachusetts LPN license. Bilingual Spanish candidates with experience in acute care settings are preferred. Ability to build positive, productive relationships with care teams, service providers, and coordinators of other formal supports, such as DDS and DMH case managers. Excellent computer and assessment skills are required. Two year grant funded position
