Eldorado Rehab and Healthcare currently has an opening for the position of Care Plan/Restorative Coordinator.

We are seeking applicants that will oversee care plans and collaborate with IDT team and Therapy for the betterment of the residents through care plans.

Requirements:

• Must have previous experience in MDS, Restorative nurse or Infection Prevention.
• Must have a LPN or RN license (RN preferred)
• Must have a current CPR card

The duties of the position are the following:

• Creates and updates care plans for all residents on a ongoing basis
• Schedules care plans with residents/responsible parties based on MDS calendar and as needed
• Coordinates and schedules 48 hour care plans on new admissions
• Keeps IDT abreast of scheduled care plans
• Leads and participates in all care plan meetings
• Collaborates with assigned MDS coordinator on a routine basis
• Coordinates restorative services; works closely with DON, MDS, and Therapy in regards to programming, assigns tasks to restorative CNAs, audits services and maintains restorative tracking log
• As a nurse manager, participates in weekend on call rotation as well as weekend manager rotation
• Supports the Director of Nursing and nurse management team as needed/assigned
