Responsibilities:
- Assess and evaluate patients with wounds, identifying individual needs and developing customized care plans.
- Coordinate and communicate with healthcare providers to ensure seamless and effective wound care.
- Monitor and document wound healing progress, adjusting care plans as needed.
- Educate patients and their families on proper wound care techniques, prevention strategies, and lifestyle modifications.

- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid state IL license.
- Certification in Wound Care (WCC) is highly desirable.
