Position: Case Manager RN - Home Health
Shift: 8:30a-4:30p, 1 weekend per month
Salary: $33 - $43 p/hr Mileage Reimbursement
Position Information:
Our client is seeking a full-time RN Case Manager for Home Health in East Cuyahoga County, OH. The ideal candidate will provide skilled nursing care, demonstrate effective communication and collaboration, and maintain patient safety. This role requires strong organizational skills, the ability to work independently, and a commitment to maintaining confidentiality. The candidate should have experience in home health care. This position will service the following coverage areas; Bratenal, East Cleveland, Cleveland Heights, University Circle and surrounding areas. EMR: Home Care Home Base Equipment Provided: Tablet, Phone
Travel: 5-7 visits/per day (Dependent on type of visit) Travel Radius: Expected to cover all counties (Not in same day)
This position is a part of a union. Rates given are the established union rates, based off of years of experience. Please see the pay rate word document attached.
Requirements

• Associate degree or higher

• 1 years of experience in ANY of the following:
  
  • Hospice
  • Acute Care
  • Emergency Department (ED)
  • Critical Care
  • Burn Unit/Wound Clinic
  • Oncology
  • Medical-Surgical (Med-Surg)
  • Home Health Care

• Valid Registered Nurse (RN) in the state of Ohio (OH)
• Valid American Heart Association Basic Life Support (AHA BLS)
