Located in the vibrant Fulton Market district of Chicago, EMMAUS is a Christian 501(c)(3) nonprofit dedicated to combating global exploitation and supporting male survivors in their journey from the sex trade to a life of freedom and fulfillment. For more information, visit www.streets.org.

Background

The COVID-19 pandemic has underscored the critical need for secure housing, particularly for those most vulnerable. Many individuals experiencing homelessness face heightened risk of COVID-19 exposure, making immediate access to housing essential. In response, the Chicago Continuum of Care (CoC) established the Expedited Housing Initiative (EHI) to address the crisis and expedite the resolution of homelessness in Chicago.

The EHI is pioneering innovative strategies to end homelessness faster by reducing barriers and increasing federal funding. This collaborative initiative includes the Chicago Department of Family & Support Services (DFSS), the Chicago Department of Public Health (CDPH), All Chicago, the Corporation for Supportive Housing (CSH), and the Chicago Continuum of Care.

All Chicago has partnered with various agencies to offer Rapid Rehousing and case management services for up to 2,000 households experiencing homelessness. If you are interested in becoming a case manager for the EHI, please refer to the application process outlined below.

Position Overview

The Case Manager will report to the Managing Director of Programs and will play a pivotal role in assisting individuals and families experiencing homelessness to secure and maintain housing. The ideal candidate will employ evidence-based practices to set client goals, identify barriers to housing stability, and provide crisis intervention. The Case Manager will also contribute to the oversight of the Drop-In Center at EMMAUS, supporting men in their journey from exploitation and aiding in the development of trauma-informed programs.

Education and Experience

  • Bachelor’s Degree in Psychology, Sociology, Social Work, or a related field required; Master’s Degree in Clinical Psychology preferred.
  • CADC, LPC, or LSW designation, or relevant “hard knock” experience preferred.
  • Strongly desired: Outreach, drop-in center, or clinical experience. Evidence of nonprofit volunteer work, especially with organizations similar to EMMAUS, is a plus.
  • Must pass DCFS requirements, including Health Clearance and Criminal Background Check as applicable.

Key Responsibilities

  • Conduct monthly or as-needed home visits to ensure housing stability, support life skills development, and provide emotional support using a trauma-informed, strength-based approach.
  • Perform crisis prevention and intervention utilizing Harm Reduction and Trauma-Informed Philosophies.
  • Document and report critical client issues to enhance housing stabilization efforts.
  • Employ strength-based and motivational interviewing techniques to help clients achieve their housing goals through external referrals for services like workforce development, financial stability, and healthcare.
  • Advocate for clients in securing governmental benefits (SNAP, SOAR) and address housing-related needs, including eviction prevention, landlord negotiations, and accessing community resources.
  • Write case notes, letters, and reports; maintain documentation and complete required forms.
  • Monitor and track client progress towards housing goals.
  • Collaborate with property owners/representatives to secure necessary documents for financial assistance.
  • Provide follow-up services to support clients in maintaining housing post-exit.
  • Enter and manage data accurately in HMIS, ensuring timely and complete records.
  • Foster a welcoming environment at the Drop-In Center and lead efforts to plan engagement activities.
  • Ensure compliance with organizational and external policies and standards.
  • Participate in supervisory, clinical, and program team meetings and training.
  • Connect clients to community resources and maintain agreements with relevant organizations.
  • Perform other duties as assigned by the immediate supervisor or designee.

Work Requirements

  • Must reside in or near Chicago and have reliable transportation for local outreach.
  • Role involves a combination of in-office work and home visits, including daytime and occasional nighttime hours.
  • Ability to navigate and address potentially sensitive online content with protective processes and technology.
  • Strong verbal and written communication skills, excellent organizational abilities, and attention to detail are essential.
