ESSENTIAL JOB RESPONSIBILITIES:
Include the following.Other duties may be assigned.

• Facilitates the coordination of patient's plan of care.
• Facilitates IDT communications and cohesiveness, ensuring a smooth transition and continuity of care throughout each shift of daily care.
• Evaluates and educates staff in assessment and Hospice philosophy in accordance with the policies and procedures of Hospice.
• Supervises and facilitates problem resolution of patient/family care and team functions.
• Conducts monthly staff meetings.

QUALIFICATION REQUIREMENTS:

• Registered Nurse with a license issued by the Florida Board of Nursing. Bachelor's degree in nursing preferred.
• Minimum of (3) three years clinical nursing experience; leadership experience and at least (1) one year hospice experience preferred.
• Current Basic life support (BLS)/CPR Certification required.
