Summary:

Reporting to the Program Manager, this position plays an essential role in providing services to individuals who are incarcerated or newly released from incarceration. The Intensive Case Manager provides basic case management services as well as Enhanced Case Management services such as assessment, goal-setting, care coordination, and crisis management. The following Evidence-Based Interventions are used: Motivational Interviewing; Strengths-Based Model; Trauma-Informed Care. The ReTurn Project is a program which offers: case management and therapeutic services to clientele. The ReTurn Project strives to provide supportive and wraparound services to its clientele in hopes of reducing the impacts our clients face related to that of co-occurring disorders, mental health disparities, and substance use disorders.

Responsibilities:

  • Provides case management to individuals who are in-custody or recently released from incarceration.
  • Provides regular case management appointments when clients are released from custody.
  • Facilitate assessments- effectively gather information using evidence-based screening tools and motivational interviewing skills in order to identify goals in the care plan
  • Manages a caseload of clients and is responsible for helping identify and meet their needs in areas such as Substance Use Disorder (SUD) treatment, housing, employment, education, physical and mental health.
  • Coordinates referrals, appointments, and meetings and attends appointments and meetings according to level of need for support
  • Effectively manage crisis by using Motivational Interviewing and De-escalation skills, both in support of patients and other team members
  • Offer exceptional care coordination skills that include warm hand-offs and closed-loop referrals
  • May transport clients to appointments in own vehicle.
  • Maintains clinical documentation of all services provided to clients.
  • Understanding of transdisciplinary teams and roles using an integrated approach to care

Qualifications:

  • Bilingual highly preferred.
  • Bachelor’s Degree in Social Work, Counseling, Psychology or another related counseling field preferred.
  • Minimum 1 year experience providing case management or counseling support to clients involved in the criminal justice system.
  • Minimum 1 year experience providing enhanced case management to clients.
  • Demonstrated work experience successfully working in collaborative teams
  • Ability to complete documentation accurately and in a timely manner
  • Strong skill set regarding clinical ethics, boundaries, and role clarity with clients
  • Experience with in-custody programs preferred
  • Applicant must have active driver’s license and the ability to drive clients.
  • Applicant must submit appropriate background check clearance and be able to pass Santa Cruz County jail clearance to provide in custody services.
