Encompass Community Supports is Hiring a Support Coordinator II - Developmental Disabilities Near Culpeper, VA

Encompass Community Supports, formerly known as Rappahannock Rapidan Community Services (RRCS) is hiring a Support Coordinator II-Developmental Disability to work in our Culpeper office. Our Support Coordinator II-Developmental Disability assesses clients with intellectual/developmental disabilities by identifying needs and providing access to appropriate services from our agency and other agencies including service linkage, case monitoring and client advocacy functions; work is performed under regular supervision.

Our full-time employees work 37.5 hours weekly.

ESSENTIAL FUNCTIONS:

  • Facilitates, assesses and evaluates client services and needs in the home, hospital or clinic setting; presents options and services based upon a needs assessment.
  • Identify individuals who meet the criteria and provide enhanced case management according to the requirements.
  • Develops an individual service plan of care and services appropriate to the evaluation with the client/family consensus; reviews and updates service plans.
  • Submit referrals to the Regional Support Team in a timely manner for review prior to the client being placed in a large congregate setting.
  • Discuss and/or develop employment-related goals or employment-readiness goals with clients during annual or semi-annual report periods.
  • Refers, links, and coordinates to multiple agencies to ensure appropriate service initiation and delivery.
  • Ensures programs operational compliance to all regulatory guidelines.
  • Prepares and maintains client charts and other records and documents all activities performed on clients’ behalf.
  • Prepares applications and variety of reports related to benefits and potential funding sources to meet client needs.
  • Advocates for clients rights, needs and access to services while ensuring human rights and satisfaction with all services.
  • Assists with discharge planning from care facilities and educational programs.
  • Provides crisis intervention and prevention services.
  • Attends meetings, staffing, and conferences as they relate to client, staff and program needs. Attends inter-disciplinary team meetings to discuss decisions for client plan of care.
  • Provides or arranges transportation.

Other FUNCTIONS:

  • Performs other related functions as required.

KNOWLEDGE, SKILLS AND ABILITIES:

  • General knowledge of community and agency resources and programs of regional and state sources for the community service population.
  • General knowledge of interviewing and supportive counseling techniques.
  • General knowledge of social casework techniques.
  • General knowledge of human development and behavior.
  • General knowledge of the theories, principles and techniques of individual, family and group therapy.
  • Ability to analyze facts and exercise sound judgment in arriving at conclusions.
  • Ability to communicate complex ideas effectively, orally and in writing.
  • Ability to prepare clear and concise reports.
  • Ability to establish and maintain effective working relationships with clients, associates and the general public.
  • Knowledge of MS Office, to include Word and Excel.

EDUCATION AND EXPERIENCE:

Bachelor’s Degree in Human Services or equivalent, and at least 1 year working with individuals with intellectual disabilities is required.

SPECIAL REQUIREMENTS:

Valid Virginia driver’s license; attends training and maintains certifications as required by the Agency and regulatory guidelines, such as CPR, First Aid, Person-Centered Plan, and DBHDS Case Management Modules.
