Position Highlights
- Position: Care Navigator- Social Worker $10,000 sign on bonus
- Location: Evanston Hospital
- Full Time/Part Time: Full-Time, 40 hours per week
- Hours: 8:00 AM- 4:30 PM and 11:30 AM-8:00 PM (rotating hours) with a rotation to cover weekend hours
- Required Travel: May travel to all 4 sites (Evanston, Glenbrook, Skokie, and Highland Park)- limited
- License: LSW or LCSW license required. CPR-BLS for Healthcare providers (American Heart Association) required.
- Education: Master’s Degree from an accredited school of social work required. Clinical certification, such as case management certification, is beneficial.
- Experience: Two years post-graduate experience preferred; healthcare experience preferred. Experience with Electronic Medical Record (EMR) platform preferred.
- Provides emotional support and counseling to the patient and caregivers
- Helps patients understand their insurance coverage as well as the post-acute level of care, necessary durable medical equipment required for recovery
- Coordinates and collaborates with inpatient continuum Care Manager while the patient is hospitalized, accepting hand off at the time of discharge.
- Completes and documents assessment in accordance with NorthShore and health plan requirements.
- Develops, implements, updates individualized care plan for patient until program completion.
- Documents in health plan portals as required.
