Represent the organization in internal transitional care activities utilizing standardized processes for identifying, engaging, and transitioning patients with life limiting illnesses who are eligible for the hospice benefit. Integrate clinical guidelines, tools, and other metrics in the development of transition plans that are patientcentered, promoting quality and efficiency in the delivery of care. Serve as a representative to bring awareness and internal education relative to hospice services available, especially the care connections program.

Education and experience, essential

• Must be a graduate of an approved school of nursing, therapy, or social work.
• Must be licensed in the state where they currently practice.
• Must have two years' demonstrated clinical experience.
• Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of a home health or hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

• Three years' demonstrated clinical experience is preferred.
• Previous experience in and knowledge of home health and hopsice practices is strongly preferred.
• Demonstrated experience and understanding of customer and market dynamics, as well as the requirements and best practices of transitional care within home health or hospice is preferred.

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
