We are looking for a motivated home health aide to provide aide services in accordance with the patient’s plan of care to include: providing personal care and related services under the supervision of nurse case managers, monitoring of the patient’s condition, health promotion and prevention coordination and services, and teaching and training activities.

Your Duties:

• Follow appropriate plan of care to provide safe and competent care to patients.
• Assist patients in maintenance of personal hygiene and healthful, safe environment.
• Assist therapy personnel with rehabilitative processes such as ambulation, mental alertness, and simple psychological support.
• Coordinate appropriate care, encompassing various healthcare personnel (such as Skilled Nurses, Physical Therapists, Occupational Therapists, Home Health Aides and external providers).
• Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
• Report changes in the patient's mental or physical condition, or in living situation to clinical management.
• Prepare visit report promptly to maintain accurate medical record.

Qualifications:

• Diploma – High School or equivalent
• Minimum of 75 hour training to include an Introduction to Personal Care Services
• Home Health experience preferred
• Current CPR-BLS Healthcare Provider certification
• Valid driver’s license
