We are seeking a dedicated Comprehensive Case Manager/Placement Specialist to join our team and help us make a difference in the lives of those we serve.

Job Description: The Comprehensive Case Manager/Placement Specialist provides comprehensive case management and supportive relationships to potential and active participants across all programs offered by Entrust. This role involves fostering positive support, advocating on behalf of individuals and families, and facilitating the development of individualized plans to reduce barriers. The Case Manager/Placement specialist monitors progress and maintains documentation of plan progress, changes, and activities, and is responsible for implementing all aspects of the Individual Program Plan (IPP), including intake, development, job training, job placement, job coaching, and program documentation. This role involves developing employer and business accounts, ensuring compliance with agency philosophy, accreditation requirements, and applicable laws.

Key Responsibilities:

  • Partner with Intake Outreach Case Aid during intake and assessment to develop and implement individual and family service plans.
  • Perform periodic reassessments, updating plans and maintaining regular communication and documentation.
  • Maintain supportive relationships with participants, agencies, and community systems.
  • Distribute completed plans, forms, and reports to team members and appropriate external agencies.
  • Ensure team members understand their assignments related to the service plan.
  • Collaborate with internal and external partners to maintain cohesive professional relationships.
  • Exit participants according to program procedures.
  • Communicate with Program Manager and service team to identify participants in need of services.
  • Ensure proper implementation of documents and procedures in program databases according to program rules.
  • Research community resources to assist with participant support needs.
  • Communicate regularly with internal staff, external agencies, care providers, and participant families.
  • Coordinate coverage of services during absences.
  • Perform other duties as assigned.
  • Perform intake, assessment, and development of IPP goals.
  • Conduct periodic reassessments and update IPPs, maintaining regular documentation and communication.
  • Facilitate ISP meetings and act as a facilitator for planning sessions.
  • Maintain participant master files and input service hours into Setworks.
  • Implement and recommend revisions to training plans and methods.
  • Educate employers and co-workers about participants' support needs.
  • Plan and implement support services schedules for participants.
  • Address and overcome barriers to employment for participants.
  • Develop business accounts and employment leads.
  • Maintain employer databases and submit progress reports.
  • Participate in sales team goal setting and policy development.

General:

  • Communicate with funding agencies, care providers, and participant families.
  • Advocate for participants' rights and privileges.
  • Coordinate service coverage in case of absences.

Education and Experience:

  • Bachelor’s degree in Psychology, Sociology, Special Education, or related field and 1-year experience; or
  • Associate's degree in related field and 2 years' experience; or
  • High School diploma or GED with 3 years' experience in Case Management and/or additional specialized human service training.

Certificates, Licenses, Registrations:

  • Current First Aid/CPR certification (preferred)
  • Valid Washington State Driver's License (required)
  • Proof of automobile insurance (if a vehicle is driven)

Other Requirements:

  • Ability to pass Washington State Criminal Background Check
  • Ability to pass mandatory drug screen
  • Ability to pass E-Verify
  • Does not pose a direct threat to the health and safety of self or others

Physical Demands:

  • Regularly required to use hands, talk, and hear.
  • Frequently required to stand, sit, walk, and reach with hands and arms.

Work Schedule:

  • 8:00 AM - 4:30 PM, Monday to Friday (1/2-hour lunch). Hours may vary.
