Position: Hospital Liaison

Job Specification:

We are currently seeking a dynamic and motivated Hospital Liaison to join our team. The Hospital Liaison will serve as the primary point of contact between our facility and local hospitals, healthcare providers, and community organizations. This role will be responsible for building and maintaining relationships, facilitating admissions, and promoting our services to potential residents and their families.

Hospital Liaison Responsibilities:

• Establish and maintain relationships with hospital discharge planners, case managers, physicians, and other healthcare professionals to promote referrals to our facility.
• Collaborate with hospital staff to facilitate timely and smooth transitions of care for patients being discharged to our facility.
• Conduct pre-admission assessments and coordinate admissions, ensuring that all necessary paperwork and documentation are completed accurately and efficiently.
• Educate hospital staff, patients, and families about our services, programs, and amenities.
• Represent our facility at community events, health fairs, and networking functions to raise awareness and generate referrals.
• Maintain accurate records of referrals, admissions, and outreach activities.
• Participate in interdisciplinary team meetings and contribute to care planning for new admissions.

Hospital Liaison Qualifications:

• Bachelor's degree in Healthcare Administration, Business Administration, Nursing, or a related field.
• Experience in healthcare sales, marketing, or admissions, preferably in a skilled nursing or rehabilitation setting.
• Strong interpersonal and communication skills, with the ability to build rapport and establish trust with diverse stakeholders.
• Knowledge of Medicare/Medicaid regulations and reimbursement processes.
• Ability to work independently and collaboratively as part of a team.
• Valid driver's license and access to reliable transportation.
