Epilepsy Alliance Ohio is Hiring a QIDP/Case Manager Near Batavia, OH

QIDP /Manager
We are seeking a highly motivated individual to join our team and make a positive impact on the lives of our adult residents working as the QIDP overseeing two of our group homes in our residential program. The QIDP will ensure the overall design, delivery, and coordination of services through person centered planning and active treatment for approximately 16 individuals living in our ICF group homes. This person is primarily responsible for training, coaching and supervising the direct care staff who are implementing individuals programs, and ensuring the health and safety of all the individuals served while working in collaboration with other team member. This position requires a flexible schedule that includes working primarily afternoon and evening hours as well as some weekends; as the QIDP is present during varied shifts to monitor programs. The QIDP is available Monday-Friday by phone to address residents behavioral issues and/or emergencies and is part of a weekend/holiday management on call rotation. The ideal candidate will be a dependable self starter who is motivated and is able to recognize needs of individuals served and be creative with skill building strategies to address the needs identified for those individuals and implement these skill building strategies into their programing. A Bachelors degree in Human Services or related field is required and one year experience working with adults with developmental disabilities and supervisory experience. Must be at least 21 years old, able to pass a State and Federal background check, has a good driving record, able to follow and maintain drug free workplace standards, and able to pass a pre-employment physical that includes lifting at least 50 pounds. Prefer individual with some supervisory experience in the DD field. Final salary will be determined by experience and education level.
