We are seeking a registered nurse (RN) to join our healthcare team. The successful candidate will provide high-quality patient care, document patient interactions, and support the clinical team.

Key Responsibilities

• Provide and document patient care, including injections and preventive screenings
• Triage patients according to approved protocols
• Act as a member of the care team, preparing patients for appointments and collecting clinical information
• Educate patients on self-management of their condition
• Carry out standing orders and document all patient interactions in the electronic health record (EHR)
• Conduct outreach to patients and follow up on appointments and test results
• Answer and document patient phone calls and messages
• Monitor task buckets and follow up on overdue orders and referrals
• Support the Clinical Lead and other clinical staff members
• Precept new team members and maintain open lines of communication with the care team and Regional Management

Requirements

• Active, unrestricted RN licensure in CT
• 1 years of nursing experience
• Intermediate level of computer proficiency in Microsoft Office

Preferred Qualifications

• BSN
• Experience with Epic
• Experience in a hospital or medical office setting

Working Conditions

• Adhere to all patient safety initiatives and infection control regulations
• Maintain knowledge of current coding, OSHA, and CLIA regulations and company policies
