High Fidelity Wraparound Division
We are seeking individuals with high energy, strengths-based recovery perspective, and dedication to helping families and youth achieve their vision. Facilitators help families use their voice, choice, and strengths to develop a support plan. This position provides support to youth with severe emotional disturbance (SED) and multiple system involvement to empower them to live successfully in the community with their families. The Facilitator will provide neutral coordination of teams, developed by youth and families, to build natural supports and self-efficacy.
SPECIFIC DUTIES INCLUDE:
Ensuring the High Fidelity Wraparound process is implemented with fidelity.
Utilizing organizational and time management skills.
Engaging and preparing the team of people who are providing services and supports to the family.
Developing with the families the Strength, Needs Cultural Discovery Document, Crisis Stabilization Plans, Family Plans of Care and Transition Plans.
Striving to teach families to manage their own services, supports and plans.
Helping youth and families strengthen natural and social support systems.
Ensuring the team mission and goals address the priority needs and develop an effective, integrated plan that focuses on these needs.
Monitoring implementation and effectiveness of plans and the process.
Determining when transition from High Fidelity Wraparound is appropriate.
Completion of required training and credentialing process for High Fidelity Wraparound Facilitators by the Youth and Family Training Institute (YFTI).
Other duties as assigned.
QUALIFICATIONS:
Bachelor’s level degree in a human service field or Bachelor’s level degree in any field with a minimum of two years of full-time experience working in relevant family, children/youth or community service capacity.
Preferred knowledge of community resources and an ability to engage these resources.
Valid driver’s license and own transportation required.
Act 33 and 34 clearances required.
