SUMMARY:

The Vice President (VP) of Population Health is a strategic leadership role responsible for overseeing and advancing the organization's population health initiatives. This role will focus on improving health outcomes, managing healthcare costs, and enhancing the quality of care for the patient population. The VP of Population Health will report directly to the Chief Medical Officer (CMO) and will be responsible for managing direct reports.

QUALIFICATIONS:

• Education: Master’s degree in Public Health, Healthcare Administration, Nursing, or a related field.

• Licensure/Certification: Preferred ANCC-recognized Care Management certifications in relevant fields such as CCM or APN.

• Years of Experience: Minimum of 10 years of experience in healthcare management, with at least 5 years in a leadership role focused on population health or care management.

KNOWLEDGE, SKILLS AND ABILITIES:

• Strong knowledge of population health principles, care coordination, and quality improvement methodologies.
• Proven ability to develop and implement strategic plans and initiatives.
• Excellent analytical, communication, and interpersonal skills.
• Experience with data analytics and health information technology.
• Ability to work collaboratively across disciplines and with diverse populations.
• Strong leadership and teaching skills to develop cohesive, creative, and successful team members.
• Knowledge in program development, implementation, and process improvement.
• Demonstrated ability to meet established targets and deadlines.
• Strong organizational skills to perform a variety of tasks independently, timely, and effectively.
