RN Home Care Coordinator

Under the supervision of the Center Administrator, the Home Care Coordinator oversees the day-to-day operations of the Home Care program. Directs and supervises the Home Care R.N., Home Health Aide, and the Personal Care Attendants (PCAs) in the provision of activities of daily living, day center activities, restorative and supportive care of the participants. Shall assist leadership in policy formation and program development and implementation. Participates in the establishment of annual budgets. Provides supervision visits to home care staff in the field quarterly to evaluate skill and performance. Performs initial assessment, annual reassessment and change of condition assessments of participants in their home environment and coordinates plan of care with appropriate resources. When appropriate, provides health education for participants in their home. Shall act as the liaison between the contracted home health agencies and personal care agencies and PACE. Will insure compliance with competency requirements. Maintains the participant’s medical record with thorough documentation. Communicates participant’s changes and progress to appropriate staff.

Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

Must have a valid driver’s license, proof of insurance and have means of transportation. Must be able to meet EveryAge's auto insurance carrier's standards for coverage.

Graduate of a school of professional nursing. Current North Carolina Registered Nurses License, Cardio Pulmonary Resuscitation (CPR) certification required, Bachelors of Science Nursing (BSN) preferred.

A minimum of one year’s experience in working with the frail or elderly population required. Minimum of two year’s nurse management experience in a hospital, nursing home or community based setting.
