**Job Overview:**

We are seeking a dedicated Licensed Practical Nurse to join our healthcare team working in a group home setting as well as the office. The ideal candidate will provide quality care to patients under the direction of physician's orders and report directly to the COO. This needs to be a minimum of 24 hours/week, but can discuss the possibility of full time.

In-Home needs:

-Medication Audits (Weekly Minimum & Ongoing)

-MAR Audits (Weekly Minimum & Ongoing)

-Occasional medical care based on a physician's order

In-Office Needs:

-Complete HRST (Health Risk Screening Tool) Online

-Medical Records Audit (Weekly Minimum & Ongoing)

-Communicate with individuals, families, DSPs & Residential Directors to schedule medical appointments (as needed) in a timely fashion according to Chapter 6400 regulations

-Communicate with Medical Providers to confirm & verify medical diagnosis, provide training to agency staff for care, and arrange for follow-up care appointments

-Create and maintain a medical appointment spreadsheet (Weekly minumum & Ongoing)

-Monitor Online MAR (Quickmar)

Job Types: Full-time, Part-time

Pay: $25.00 - $29.00 per hour

Expected hours: 24 – 40 per week
