Exodus PCS assists Nursing Facilities and SUD treatment residents locate community housing (Assisted Living, Foster Care, Independent Living) and coordinates with providers and community resources to ensure a smooth transition to their home. Qualifications:

We’re looking for a full-time Case Manager. The ideal candidate will be self-motivated, dependable, and caring. Applicants must have a 4 year degree in health/human services and experience in the health and human services field. Candidates must be able to work remotely and be able to drive throughout the Twin Cities Metro.

• Bachelor's Degree in Social Work, Psychology, Sociology, Human Services, Nursing or an acceptable human services field of studies
• At least two years experience working in the field of Human Services, Social Services, or Healthcare.
• Knowledge of community resources, waiver programs, county applications
• Experience with medical assistance, housing coordination, discharge planning, case management, CADI waiver, Elderly Waiver, service agreements and direct care
• Clearance to work in the field of human services through a background check done by the Department of Human Services
• Reliable vehicle with insurance and a valid driver's license (job requires driving your own vehicle, there is mileage reimbursement @ federal rate)
• Experience with Microsoft 365, Microsoft Teams
• Ability to lift up to 25 lbs. (day of discharge)

Location:

Exodus PCS’s main office is located in Burnsville, MN. First week of training is at main office. Then a typical day consists of remote work from home.
