Responsibilities:
- Conduct patient assessments to determine their needs and develop appropriate treatment plans
- Collaborate with other healthcare professionals to coordinate care and ensure holistic treatment approach
- Utilize case management skills to connect patients with necessary resources and support services
- Maintain accurate and up-to-date medical documentation in compliance with regulatory standards
- Stay informed about current research and best practices in the field of social work
- Attend daily interdisciplinary meetings, work with PASRR clients, and expedite care plan conferences
- Facilitate discharge planning and the recording, tracking, and resolution of resident grievances
Skills:
- Applied behavior analysis: Ability to apply behavior analysis principles to support individuals with behavioral challenges
- Case management: Strong organizational and coordination skills to effectively manage client cases and connect them with appropriate resources
- Psychopharmacology: Understanding of psychotropic medications used in mental health treatment
- Medical documentation: Proficiency in maintaining accurate and thorough medical documentation, including familiarity with ICD-10 coding system
- Research: Ability to stay updated on current research findings and incorporate evidence-based practices into treatment plans
- Patient assessment: Skill in conducting comprehensive assessments to evaluate patients' mental health needs and develop appropriate treatment plans
- Crisis management: Ability to properly assess client needs and work with the interdisciplinary team to resolve any issues
