Extended Care Management LLC is Hiring a Director of CL Services IN Near Greenwood, IN

ESSENTIAL DUTIES AND RESPONSIBILITIES: The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive, other duties may be assigned:

Team Leadership/Supervision

  • Oversee the clinical performance and achievement of productivity expectations of assigned Care Teams
  • Foster an engaging culture among all teammates
  • Manage adherence to established and documented Eventus clinical standards, processes and procedures. Receive and react to clinical quality data reports
  • Collaborate with physicians, clinical leaders and/or director of clinical services when specialty specific guidance is needed. Act as clinical and workflow expert for clinical leaders within the team and help facilitate the resolution of clinical inquiries from clinical leaders, providers, facility personnel and family members

Regularly meet with direct reports to assess their job performance as well as mentor, guide and train on any job duties that are not being met

  • Collaborate with the care team administration and state operators to ensure metrics are met
  • Assist with creation, development and organization of care team, state and company meetings as well as new company initiatives
  • Identify and report any process improvements that could be made within clinical and quality areas

Care Team Clinical Integration

  • Ensure adoption of Eventus clinical model in all available facilities, providers and care teams with the ability to demonstrate and promote the value of the clinical model in both written and spoken language
  • Receive and analyze clinical quality data and team and provider productivity reports; reacting and counseling team members, as needed, to maintain standards
  • Promote interdisciplinary collaboration through coordination with all service lines
  • Keep team members informed of all recommendations for process improvements, clinical issues, employee, facility customer or patient/family issues
  • Create and maintain a team spirit and positive patient focused culture among teammates

Growth and Retention

  • Ensure external excellent customer service through retention of existing facilities, both in numbers of facilities and maintaining/adding to existing patient caseloads through the growth of existing caseloads and addition of service lines within each facility
  • Expand Eventus clinical model of care through new facility starts utilizing the interdisciplinary and collaborative model/process
  • Ensure internal customer service through high provider and employee retention rates; recognizing positive and negative retention trends and offering solutions to improve metrics
  • Assist with expansion, not only in new areas of business/facilities but also in the design of new care team organization/future roles not yet realized

Clinical Services

  • Perform clinical services within the standards of Eventus WholeHealth clinicians for area of clinical specialty on as needed basis
  • Provide backup and fill-in clinical services (e.g., PTO and uncovered buildings coverage) when needed and work alongside providers in trainings or starting up new buildings, to ensure that all patients within the care team are serviced.

EDUCATION AND/OR CERTIFICATIONS

  • Clinical Masters or Doctoral Degree
  • National certification for area of specialty
  • Unrestricted license in area of specialty within the state of practice

SKILLS AND QUALIFICATIONS

  • Previous or current long-term care experience required
  • Experience leading integrated healthcare teams a plus
  • Demonstrated experience with an Electronic Health Record (EMR) and/or practice management system
  • Excellent PC skills required
  • Strong leadership skills and the ability to effectively coach, motivate, and develop others.
  • Excellent oral, written and interpersonal communications skills, with the ability to bring individuals together.
  • Ability to work collaboratively and coordinate care with a multi-disciplinary team Ability to effectively handle multiple priorities
  • Possess outstanding organization skills and an ability to multi-task while maintaining a customer focus
  • Demonstrate creativity, problem-solving, adaptability, and flexibility
  • Knowledge of HIPAA privacy laws
  • Must possess an understanding and adherence of legal and regulatory agency rules and guidelines
  • Drive to help patients and staff with tact and diplomacy

LANGUAGE SKILLS:

  • Ability to communicate clearly and concisely in verbal and written form.
  • Strong communication skills and a customer service attitude.
  • Must be able to frequently communicate with facilities and patients and/or with others about patient or provider requirements, complaints, and common inquiries.
