Status: Full Time; Days; M-F

Nature of Work: Responsible for delivery of social services in the Skilled Care and Acute Care departments. Conducts case management services, provides counseling, and recommends functions necessary to ensure overall operational viability. Maintains performance improvement activities within the department and participates in CQI activities. Participates in resident MDS reviews and completes all assessments as required within required time frames. Initiates Discharge Planning upon admission providing education based upon the patient’s needs throughout the patient’s stay. Documenting all interventions Manages & carries out the Social Services department program in relation to Social Services for FMH in accordance with accepted Social work practice while providing educational resources regarding community and agency information to referred residents, patients, or families

Qualifications/Requirements: A Bachelor’s Degree in Social Work is preferred from an accredited School of Social Work or a Bachelors in a related field, such as Psychology/Sociology or Nursing with 2 years of experience. Two (2) years of supervised social work experience in a health care setting is preferred, working directly with individuals. Must possess a working knowledge of Illinois Department of Public Health regulations.
