Faith Presbyterian Hospice is Hiring a LVN Field: Friday 5pm-12am and Saturday and Sunday 8am-8pm Near Dallas, TX
Position Description: ESSENTIAL DUTIES: Models a service attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co-workers Demonstrates appropriate skills for initial and follow-up appraisal of patient status. Possesses an adequate knowledge of clinical diseases Demonstrates ability to perform nursing treatments and procedures as prescribed for patients assigned. Administers those treatments in an appropriate manner Demonstrates adequate knowledge of medications commonly prescribed to patients and administers appropriate medications Demonstrates knowledge and awareness of palliative aspects of care and ensures patient safety, comfort and protection Ensures that all residents under his/her direct supervision is provided care and services to attain or maintain the highest practical quality of life. Ensure resident/patient, visitor and co-worker safety by remaining aware of and reporting any new or existing safety hazards Maintain a good attendance record Competently perform all basic and assigned nursing skills within a licensed vocational nurses scope of practice Actively participate in all aspects of patients care, in accordance with current professional standards and practice Follow established standards of nursing practices and implements Faiths policies and procedures to provide quality patient care Conducts patient visits/continuous care shifts as schedule by the plan of care, reports problems and communicates appropriate patient information to RN Case Manager Assists with any activities as related to patients and deaths Practices standard infection control techniques Notify the Medical Director, family, administrative staff and direct care providers of patient change in condition and necessary follow up Documents notes on each patient reflecting nursing care, patient instruction and patient/family response, doctor notification, a specific plan for ongoing care Communicates unresolved work-related problems using chain of command, consistently demonstrates effective problem-solving skills and handles confidential information appropriately Demonstrates flexibility with a changing workload/caseload sets priorities accordingly and carries out job assignments as directed. Consistently maintains productivity at an acceptable level established by Faith Hospice Other duties as assigned by Patient Care Manager, RN Case Manager or designee Additional Information: MINIMUM QUALIFICATIONS: Valid Texas License Vocational Nurse license Previous acute care, long term care, hospice or home health 1 year experience preferred Knowledge of current professional nursing theory and practice Excellent technical and documentation skills Ability to organize work flow, respond to emergencies and note detail Ability to maintain a positive, stable disposition in performing work in association with staff and residents. Ability to maintain a high level of performance in an environment of frequent demands and changes Excellent verbal and written communication skills Ability to perform Essential Duties as outlined below Genuine caring for and interest in the care of mature adults, handicapped people and hospice Patients Basic computer knowledge required (Internet, word processing, database entry) Must have reliable transportation, maintain auto liability insurance on personal vehicle Must maintain current drivers license and be able to drive to assignments Ability to perform work tasks within the Physical Demand Requirements as outlined below Ability to comply with the Resident/Patient Bill of Rights and Teammate Responsibilities Ability to comply with organizational and departmental safety policies and procedures
