NIGHT SHIFT AND PRN AVAILABILITY
JOB SUMMARY: The Registered Nurse is responsible for the delivery of safe patient care utilizing the nursing process of assessment, planning, implementation and evaluation. Using leadership skills and the nursing process the RN strives to meet the needs of the patient/significant others in a safe, caring, professional manner. Provides for the well being of the patients through patient advocacy, education, and effective collegial communication with physicians, other departments and staff. Responsible for maintaining and improving clinical skills through professional development and serves as a role model for others. Performs responsibilities within the scope of practice, Hospital/Nursing Policy, State, regulatory and accreditation standards. Is able to respond quickly and accurately to changes in condition and/or response to treatment. The Registered Nurse is directly responsible and accountable for the care given to his/her assigned patients. Maintains patient privacy and confidentiality.
EDUCATION, EXPERIENCE, TRAINING
  1. Current and valid state RN License.
  2. Current BLS (AHA) certificate upon hire or obtain within 30 days.
  3. Current ACLS (AHA) certificate preferred but not required.
  4. Experience in acute care setting preferred.
  5. Experience in Behavioral Health preferred.
  6. Associate's Degree in Nursing required; Bachelor's of Science in Nursing (BSN) preferred.
  7. Behavioral violence prevention certification as required and updated yearly
DUTIES AND RESPONSIBILITIES

Recognizes signs of abuse and reports appropriately to the hospital social worker and/or Child/Adult Protective Services. Familiar with the EMTALA Regulations.

Assures effective unit operations by prioritizing patient care, problem solving and fostering collaborative working relationships.

Collaborates with physicians and the interdisciplinary team daily to assist with planning, implementation, evaluation and revision of the Plan of Care for each patient. The Plan of Care is discussed with and developed as a result of coordination with the patient/family/significant others, when appropriate.

Responsible for direct data collection and data assessment of the patient's condition. Supervises indirect data collection of assigned personnel.

Responsible for the comprehensive initial assessment and ongoing reassessment of the patient's condition, stability and determination of nursing diagnosis.

Collaborates with Therapists/Social Workers to provide optimal and efficient case management by actively participating in case management discussions.

Collaborates with Admissions Coordinator and interdisciplinary team across other departments as necessary to coordinate and expedite admissions, transfers and discharges to provide for efficient patient flow.

Completes medication reconciliation and assists with follow-through with medication orders/refills.

Collaborates with attending Physicians and/or Primary Care Physicians.

Administers injectable medications and conducts blood draws as indicated. Uses two patient identifiers consistently when drawing lab, administering medications and performing procedures. Follows medication reconciliation policy consistently.

Notifies Program Director immediately of significant problems or unusual incidents and when appropriate takes action to intervene. Communicate follow-up to Coordinator.

Monitors attendance (including tardiness) and documents/communicates to Director.

Performs according to clinical requirements of the RN when assigned or necessary to provide patient care.

Assesses and recognizes the effects and side effects of medications and documents according to established procedures.

Follows established procedures for safe medication administration.

Functions as an advocate in supporting patients' rights within the health care system.

Responsible for the observation and direction of patient lifts, mobilization, and shall participate as needed in patient handling.

Attends and maintains mandatory training in safe patient handling, trained in safe lifting techniques, includes but not limited to:
1) Appropriate use of lifting devices and equipment
2) Five areas of body exposure: vertical, lateral, bariatric repositioning and ambulation.
3) Use of lifting devices to handle patients safely.

Uses professional judgment and clinical assessment in safe patient handling that are consistent with Safe Patient Handling policies.

Complies with all regulatory requirements for clinical pathways, clinical assessments, interventions and documentation, as appropriate.

Gives thorough, detailed efficient change of shift report to ensure consistency in the plan of care.

Utilizes the National Patient Safety Goals in the work place as related to job performance. Demonstrates awareness of a safe environment for staff, patients, visitors, or any others on the unit.

Performs all other duties as assigned or required.
