Job Description

Home Health Aide

A Home Health Aide (HHA) provides essential personal care and support to patients in their homes, ensuring their safety, comfort, and well-being. Their primary responsibility is to assist patients with daily living activities and health-related tasks under the direct supervision or plan of a healthcare professional.

Responsibilities and Duties

  • Takes and records vital signs, and weighs patient as assigned
  • Prepares meals
  • Gathers biophysical, psychosocial, spiritual, cultural, and age appropriate information from patients, including information on the signs/symptoms of abuse or neglect
  • Maintains clinical records, signs each record and makes significant comments
  • Assists with simple, non-sterile dressings, testing urine, measuring and recording intake and output, using devices to aid in normal living activities, and teaching self-care hygiene
  • Meets or exceeds established productivity standard while maintaining quality patient care
  • Provide safe and direct personal care to patients in the home setting under direct supervision of appropriate professional
  • Personal Care – Perform and/or assist patient with personal care activities, such as, baths, showers, skin and hair care, and oral hygiene
  • Patient Observation – Notify RN of any changes in patient condition in a timely manner and document appropriately
  • Documentation – Record all required information on patient chart completely and accurately

Experience

  • Must have BLS certification through the AHA
  • Must have clean driving record and valid auto insurance and reliable transportation
  • Provides personal care, rehabilitative, support services, observation of patient through written and verbal communication
  • Assists with quality improvement/quality assurance activities
  • Orienting new Home Health Aides when requested or required
  • May perform additional tasks/procedures only if specific and adequate training has been given and documented by the supervisory Registered Nurse and allowed by state regulations, including providing nutritional support as needed
  • Ability to lift at least 50 pounds
  • Availability to work on weekends, evenings, and holidays
  • Prior home health experience preferred

Qualifications for home health aide:

  • State Approved Certified Nurse Aide Program AND
  • Requires minimum of 2 year of experience working under the direction of an RNor HHA Course, CNA, STNA
  • This position requires a current Home Health Aide Certificate if required by the state in which the aide is employed successful completion of competency evaluation
  • A minimum of six months experience as an aide/personal care assistant in a health care setting or completion of a training program with a minimum of 75 hours instruction is required
  • Must have successfully completed a state established or other training program and competency evaluation program or state licensure program that meets the federal regulation for home health aide services.
  • Ability to work in a multicultural, multi-ethnic environment

Language Skills:

  • Able to read, analyze, and interpret reports, general business directives, policy and procedure statements, and governmental regulations.
  • Able to write reports and correspondence at a professional level.
  • Able to immediately respond to common inquiries and complaints from employees and regulatory agencies.
  • Able to effectively communicate with individuals from diverse backgrounds and varying educational levels.
  • Able to effectively organize and present information and respond to questions from employees and external groups of varying sizes.
