Position Details

The Professional Care Manager & Counselor works autonomously, using discretion and independent judgement to provide clients, families, or vulnerable populations with medical and psychosocial support needed to cope with chronic, acute, or terminal illnesses. Includes administering client assessments, care planning, advising families care givers, providing patient education and counseling and making necessary referrals for other social services. Counselor specific duties to include individual or family counseling.

Job Duties

  • Complete a thorough assessment of each client and develop a care plan tailored to the client’s needs
  • Advocate for clients or patients to resolve crises.
  • Collaborate with other professionals to evaluate patients’ medical or physical conditions and to assess client needs.
  • Ability to demonstrate strong clinical counseling skills
  • Refer patient, client, or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education.
  • Counsel clients and patients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life.
  • Utilize consultation data and social work experience to plan coordinate client or patient care and rehabilitation, following through to ensure service efficacy.
  • Coordinate with discharge planners.
  • Organize support groups and counsel family members to assist them in understanding and/or processing life transitions.
  • Modify plan of care to comply with changes in clients’ status.
  • Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan.
  • Identify environmental impediments to client or patient progress through interviews and review of patient records.
  • Refer VP services to individuals in need of assistance.
  • Accurately document, chart and record clients billing information, billable hours, and client notes.
  • Successfully complete all billable expectations and billing requirements.
  • Participate in legal proceedings (depositions, court appearances) as needed.
  • Travel as required

Qualifications

Degreed and License/Certification without restrictions:

  • Licensure to provide direct counseling
  • Licensed Social Worker or Mental Health Counselor
  • Gerontology
  • Social Work (BSW/MSW)
  • Minimum of three years direct clinical treatment experience
  • Minimum two years’ experience in community-based Healthcare (home health, public health, home care)
  • Current driver’s license and auto insurance and the ability to drive to and from appointments
  • Certified Care Manager (required within 1 year of employment)

Compensation: Family First is committed to providing market competitive, equitable pay to all employees

Compensation: $32-45/hr
