DOMESTIC VIOLENCE HOUSING CASE MANAGER
GENERAL DESCRIPTION: Domestic Violence Housing Case Manager is responsible for providing case management and supportive services to clients fleeing domestic violence and either residing at the shelter or experiencing housing instability. The primary goal is to assist clients in addressing the barriers to maintaining permanent housing using a strengths-based, housing-first approach. Functions performed include intake, assessment, application to eligible funding program, assistance with locating housing, goal planning, connecting client to community resources, landlord engagement, and problem solving with other case managers. Data will be entered into HMIS Comparable database. Work is accomplished by completing assessment, developing an action plan including advising and motivating participants to set and accomplish housing goals with supportive services. Follow-up activities include completing exit documentation and regular and consistent communication attempts.
This position is a grant-funded and contingent upon the funding associated with the respective grant, although the grant is believed to be stable and permanent. Desired hours to be worked will be: Mon-Fri. 9 am - 5 pm with flexible hours to support client needs.
ACCOUNTABILITY: Reports to Victim Services Director
WORK SCHEDULE: Up to 36-40 hours weekly. Currently Office closes Friday at 12:00, but staff is expected to be available and respond until 5:00. Activities will be performed at the shelter and FGC office
REQUIRED: a valid unrestricted NC Driving license and reliable personal transportation
QUALIFICATIONS:
Education: Experience in Human Services field preferred. Associate degree or higher in Human Services field preferred.
