General Function:
The Population Health Outreach Specialist (PHOS) is responsible for providing contact and assessments to Health Plan assigned/ attributed members based on agreements outlined with Michigan Community Health Network (MCHN). PHOS is in charge of encouraging patient and provider shared decision-making, assisting individuals to reduce barriers to needed health care, promoting wellness through emphasis on keeping scheduled medical appointments, furthering patient self-management skills, and linking the clinical care provided by the Health Center with community based social supports. This position effectively educates, motivates, and supports patients and health plan members in pursuing behaviors and lifestyle modifications that improve health status and quality of life. All responsibilities will be performed according to best practices and reflecting the mission and vision of Baldwin Family Health Care (BFHC).
Preferred Qualification (not required) - Bilingual (Spanish/English)
