Position Summary

A member of the quality team, the Quality Coordinator will work with members to close quality gaps in patient/client care. This role will provide patient/client outreach followed by supplemental data submission to various Value-Based Contract (VBC) portals and Epic. 

Essential Job Duties and Responsibilities

The essential functions include, but are not limited to the following:
  • Attend and actively participate in all Value Based Contract meetings, trainings & patient outreach. 
  • Access each VBC portal at least monthly. 
  • Maintain the monthly scorecard for measures being tracked for each VBC. 
  • Assure optimal submission to each VBC
  • Work closely with the Quality Assurance Nurse to analyze outcomes and develop clinical processes as needed to improve the delivery of quality patient care. 
  • Communicate with Clinical Staff the needs of our patients/clients. 
  • Schedule Wellness Appointments
  • Monitor UDS Dashboard. 
  • Enter results into Epic to close patient/client gaps in preventative healthcare. 
  • Access CHIRP and enter into Epic. 
  • Work with Quality Team to develop 1-2 PI projects for the Quality Department. 
  • Communicates in a manner that is understood by patient/clients avoiding technical terms or descriptions. 
  • Courteous, professional and responsive to others in person, on the phone, or via electronic communications. 
  • Self-motivated with the ability to work independently and productively with general oversite.
  • In partnership with HR, support the leadership team in ensuring equitable experiences, strengthening inclusion, and promoting a culture of inclusivity and belonging that embraces the contributions of all team members. 
  • Actively engaged in making the Family Health Center the provider of overall healthcare and employer of choice by being a positive force promoting teamwork and harmony.

Minimum Qualifications (Knowledge, Skills, and Abilities)

  • High School Diploma required
  • 2 years: experience in healthcare required. 
  • Excellent organizational skills with the ability to manage multiple priorities
  • Experience with managing Value Based Contracts with a strong knowledge of quality management systems.
  • Proficient with excel, Teams, outlook and quality management software. 
  • Exceptional communication skills, both written and verbal. 
  • Attention to detail and a high level of accuracy in work. 

Physical Demands and Work Environment

  • The Quality Coordinator’s work involves remaining in a stationary position most of the time in front of a computer, using a keyboard, mouse, and telephone. Regularly operate a laptop and occasionally move about the work site access other office equipment. Communicate or exchange information with co-workers and other individuals in person or electronically.
