Position Summary:

Family Rise Together is seeking a dynamic and self-motivated individual with a compassionate demeanor, strong trauma-informed approach, and cultural sensitivity to fulfill the role of Scattered Site Housing Coordinator. This position entails extensive engagement with properties and caretakers as it involves roving responsibilities. The primary objective is to empower clients towards independence while ensuring continued support. Clients reside in privately owned apartments, where they are educated on tenant obligations and regularly liaise with supportive services on their path to self-sufficiency.

The Scattered Site Housing Coordinator will work closely with landlords to ensure prompt payment of rent and utilities, while also providing direct assistance to clients in achieving their personal goals towards healing and stability. The core focus of this role is to deliver Housing Stabilization Services and Housing Support Services to individuals receiving housing assistance through Hennepin County, particularly in scattered site apartments. Responsibilities encompass housing search, fostering positive landlord relations, and proficient subsidy management.

Operating within the framework of Housing First principles, our program aims to facilitate access to housing that aligns with each individual's unique needs. Through dedicated resources and unwavering support, clients are empowered to assume control over their lives and progress towards sustained stability.

Essential Duties and Responsibilities

  • Identify and resolve issues promptly and professionally or collaborate with the maintenance or management office team when necessary.
  • Communicate and collaborate with property owners and caretakers to optimize job efficiency.
  • Lead by example in maintaining excellent client relations and service standards with owners and residents.
  • Identify maintenance needs and collaborate with owners and the maintenance team for resolution.
  • Ensure compliance with Fair Housing laws.
  • Conduct regular in-home and community visits for individuals receiving Housing Support services, addressing progress, providing support, and coordinating referrals and transportation as needed.
  • Cultivate relationships with landlords, property managers, and owners willing to provide housing for participants, and assist with rental assistance administration.
  • Maintain records with the Minnesota Department of Human Services and assist individuals with budgeting and personal obligations when necessary.
  • Coordinate services effectively with case managers, economic assistance, St. Louis County Housing, and other service providers.
  • Maintain program files and records, and complete required reports.
  • Attend staff meetings, housing meetings, and supervision sessions as required.
  • Adhere to HIPAA and Data Privacy Practices.
  • Occasionally work outside regular business hours to meet program and individual needs.
  • Conduct comprehensive housing searches for clients within the scattered site apartment model.
  • Establish and maintain positive relationships with landlords to facilitate successful tenancy arrangements and promptly resolve housing-related issues.
  • Collaborate closely with clients to develop individualized support plans tailored to their needs, goals, and circumstances.
  • Provide ongoing support and guidance to clients in managing tenancy responsibilities, including rent, utilities, maintenance, and lease agreements.
  • Oversee the timely disbursement of housing subsidies and financial assistance, ensuring compliance with program guidelines.
  • Conduct regular home visits and assessments to monitor client progress and implement interventions as needed.
  • Advocate for clients to access additional support services and community-based programs promoting stability and self-sufficiency.
  • Maintain accurate records of client interactions and program outcomes using designated databases.
  • Participate in team meetings, training, and professional development activities to enhance housing support services.
  • Collaborate with interdisciplinary teams and external partners to ensure coordinated care for clients.

Qualifications (Knowledge, Skills, and Abilities)

· Must pass a Department of Human Services background study and sexual exploitation screening.

Job Type: Full-time

Pay: $20.80 - $25.05 per hour
