Position Summary
FSGC’s Care Navigator is responsible for the coordination of care between the person served and
their service providers within FSGC and in the community. This position will also be responsible
for initial collection of National Outcomes Measures (NOMs) and ensure prompt entry for accurate
reporting. This position will serve on an outpatient care team, and will work closely with FSGC
clinicians and other providers and resources in the community. This position requires an individual
to be personable and professional with a wide range of people with different backgrounds,
accurately recording data and assisting patients and their families with other services as directed
by their treatment plans.
ESSENTIAL FUNCTIONS:
• Work with the person served and families to gather and complete NOMs questionnaires and gather
other relevant data at admission.
• Submit NOMs data for database entry in a timely manner.
• Complete required documentation and assessments at admission.
• Collect height, weight, blood pressure, and pulse during admission.
• Collaborate with other team members regularly.
• Assure that information pertaining to the person served is communicated to key staff prior to
and after appointments.
• Confirm that the person served has all required information to complete next steps in treatment
prior to leaving the building after initial appointments.
• Facilitate collateral communication between the person served, support staff, direct care
providers, and the community to assist in meeting all health-related goals.
• Active and timely participation in all administrative and treatment team meetings for purpose of
case review, case assignments, and individual planning. Care Navigators’ caseloads shall be
assigned and monitored by appropriate team leaders.
• Documentation is timely, current, and accurately reflects the services being provided and the
person served response to those services.
• Act as an advocate for children/youth and families. This includes identifying gaps and
developing proposals for meeting identified needs and accessing needed resources and services.
• Maintain collaborative relationships between agency departments and community resources.
Participate in internal and external networking activities designed to increase the likelihood of
meeting the client and family's identified needs.
• Complete required training for position.
• Respect and demonstrate ability to maintain client confidentiality.
• Act as an ambassador of Family Service and Guidance Center in the community setting.
• Support and actively promote the vision and mission of Family Service and Guidance Center and
the philosophy of the strengths perspective.
• Other duties as necessary or required for the fulfillment of the CCBHC program, SAMHSA grant, or other strategic initiatives.
MINIMUM QUALIFIATIONS:
• Bachelor’s Degree in human services field or be equivalently qualified by work experience or a
combination of work experience and education.
• Familiarity with the Community Mental Health System in Kansas.
• Experience working with children and families preferred.
• Experience with interviewing and the admission process preferred.
