SUMMARY
Family Service League is seeking a full-time Case Manager for the Children’s Health Homes program. The Case Manager will offer care coordination to children and young adults with mental illness and/or chronic medical conditions. The Case Manager will be responsible for conducting meetings (both in-person and telehealth) with clients to develop strength assessments and individualized planning, as well as support them with their goal achievements. Evening hours are required for at least 2 days a week. Travel is required.

QUALIFICATIONS
Bachelor’s Degree in Social Work with 2 years of related experience or Master’s Degree with 1 year of related experience required.
Excellent interpersonal and organizational skills required.
Strong verbal and written communication skills required.
Ability to solve problems and work as part of a team required.
Proficient computer skills, including Microsoft Office, required.
Valid and clean NYS Driver’s License required.
Bilingual in Spanish is preferred.
