**Job Summary:** We are looking for a dedicated Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will play a crucial role in coordinating patient care and ensuring optimal outcomes for our patients. This position offers a rewarding opportunity to make a difference in the lives of patients and contribute to the overall quality of care provided. **Duties:** - Coordinate and manage patient care plans in collaboration with healthcare providers. - Conduct patient assessments to determine care needs and develop individualized care plans. - Monitor patient progress and make adjustments to care plans as necessary. - Communicate with patients, families, and healthcare team members to ensure continuity of care. - Utilize medical records and documentation systems effectively to track patient information and progress. - Work closely with Level II Trauma Center staff, Operating Room (OR) personnel, Hospital Medicine teams, and other departments to ensure comprehensive patient care. - Provide support in Level I Trauma Center settings, Emergency Room (ER), and Intensive Care Unit (ICU) environments. - Conduct thorough patient assessments, including physical examinations and health history reviews. - Deliver exceptional patient service by addressing patient needs promptly and professionally. **Experience:** - Previous experience as a registered nurse in a case management role is preferred. - Proficiency in patient assessment techniques and care planning strategies. - Familiarity with medical records systems and documentation practices. - Experience working in Level I or Level II Trauma Centers is advantageous. - Background in Emergency Room (ER) or Intensive Care Unit (ICU) settings is beneficial. - Strong interpersonal skills and the ability to collaborate effectively with multidisciplinary teams.
