We are seeking a compassionate and dedicated Registered Nurse to join our Hospice Team in the State College, PA and Surrounding areas. The ideal candidate will have a strong clinical background and a passion for providing exceptional end-of-life care. Our team is committed to providing personalized support and comfort in the familiar surroundings of patients' homes.

What You’ll Do

  • Provide skilled nursing care to hospice patients in their homes or in a hospice facility
  • Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team
  • Administer medication, monitor vital signs, and manage symptoms to ensure patient comfort
  • Educate and support patients and their families regarding end-of-life care and symptom management
  • Coordinate care with other healthcare professionals and community resources
  • Offer emotional support, counseling, and education to patients and their families, addressing their unique needs and concerns

About You

  • You should have an active RN license in the state in which you are practicing, or be a new grad with eligibility to take NCLEX.
  • Minimum 1 year of experience in hospice, palliative care, or related field preferred but not required.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work collaboratively in a multidisciplinary team.
  • Compassionate and empathetic approach to patient care.
  • Knowledge of hospice regulations and practices preferred.
