Responsibilities:

· On-call RN to answer phone calls from community-based hospice patients and triage their needs as needed

· Answer questions from patients and family during evening, overnight and weekend hours

· Emergency visits, comprehensive admission visits when needed and pronouncement visits. All visits are paid at a predetermined per visit rate

· Implement and update care plans to address patient’s nursing needs

· Educate and support patients, families and staff on nursing care and patient needs

Requirements:

· Active and valid NJ RN or multi-state license

· Preferred minimum of 2 years’ experience in hospice or palliative care but willing to train the right candidate

· Ability to collaborate within an interdisciplinary setting

· Willingness to learn about hospice philosophy and principles of death and dying

· Valid driver’s license and reliable, insured transportation with New Jersey state requirements

Skills:

• Exceptional communication and interpersonal skills
• Robust critical thinking and problem-solving abilities
• Capable of working independently and collaboratively in a multidisciplinary team
• Proficient in electronic medical record systems and relevant software applications
