Job Summary:
We are seeking a dedicated Care Coordinator to join our team. The Care Coordinator will be responsible for coordinating care services for individuals with developmental disabilities, ensuring their well-being and quality of life.

Duties:
- Coordinate and schedule care services for individuals with developmental disabilities
- Assist in meal preparation and medication administration
- Maintain accurate records of care plans and client progress
- Collaborate with healthcare providers, families, and caregivers to ensure comprehensive care
- Monitor and evaluate the effectiveness of care plans
- Provide support in activities of daily living as needed
- Ensure a clean and safe environment for clients

Qualifications:
- Experience in long-term care, assisted living, or home care settings
- Knowledge of systems for working with people with developmental disabilities
- Strong drive to provide compassionate care
- Ability to work collaboratively in a team environment
- Excellent communication and organizational skills
