Patient Care Coordination

  • Identifies evidence-based practice/ research findings with potential implications for changing clinical practice.
  • Clarifies and co-signs all physician orders.
  • Notifies physician of patient condition, changes or requests.
  • Prioritizes patient and staff needs.
  • Delegates responsibilities to appropriate team members.
  • Assesses the patient.
  • Plans patient care.
  • Evaluates effectiveness of plan of care.
  • Instructs patient/family.
  • Documents patient care.
  • Collaborates with other health team members.
Implements Patient Care
  • Administers medications, treatments.
  • Conducts and interprets hearing and vision tests, drug and alcohol tests, pulmonary function tests and fingerprinting all with appropriate training/certification.
  • Provides care and follow up for BWC patients, their employer and their MCO’s.
  • Delivers care for patients.
Information Processing and Documentation
  • Communicates patient information to care givers.
  • Conducts or assists with training and education classes / health screenings.
  • Documents information.
  • Transcribes and notes physician orders.
  • Charges for equipment, supplies, and treatment.
  • Provides an efficient and concise report.
  • Assists Manager with Q.A. data collection, documentation, and interpretation.
Care of Environment & Equipment
  • Maintains unit.
  • Maintains equipment.

What You Will Need
  • Must possess a current license as a Registered Nurse from the State of Ohio. Must be a graduate of an accredited program of professional nursing.
  • Must possess current hospital experience (within past five years) or be a new nursing graduate (within past two years). Nurses who do not meet this requirement must show proof of successful graduation from an approved refresher course.
  • Must be currently certified in CPR (and recertified bi-annually).
  • Must be knowledgeable regarding the nursing process and its application, including the assessment, planning, implementation, and evaluation of nursing care.
  • Prefer minimum of one year’s experience in Occupational Health nursing. Nurses who do not meet this requirement must obtain experience through education and successful passing of certifications.
  • CHC Specific Requirements: Unit-specific requirements must be obtained within 12 months of hire, if not current at the time of hire.
CHC BLS, AED

Drug screen collector certification (Urine, Hair and Saliva
BAT – Breath Alcohol Technician certification
NIOSH PFT Certification
CAOHC Audiometry Certification
Works with well, injured or ill patients, including emergency situations.
  • Works independently as well as functioning as a team member.
  • Capable of adapting to changing environment.
  • Represents FRMC to the community. Acts as an ambassador and advocate for FRMC.
  • Ability to understand and effectively communicate in both written and oral form.
  • Ability to read and write small letters and numbers and perform complex arithmetic.
  • Ability to maintain and project a positive impression of the Medical Center at all times.
  • Punctual attendance at assigned work location is required.
  • Ability to work in a safe and efficient manner and maintain an accident free work place, including ability to demonstrate a working knowledge of Medical Center emergency codes.
  • Ability to comply with OSHA regulations and CDC standard and transmission-based precaution recommendations and to utilize proper personal protective equipment. Ability to comply with provisions of applicable S.D.S. forms.
  • Successful completion of a ninety (90) day probationary period.
