**Overview:**
We are seeking a dedicated Case Manager to join our team. The ideal candidate will have a strong background in social work, crisis intervention, motivational interviewing, workforce development, addiction counseling, behavioral health, crisis management, supervising, Medicare knowledge, and mentoring.
**Duties:**
- Provide case management services to clients in need of support and guidance
-Proven work experience in case managemnt, including but nit limited to nursing, medical, mental health, care management
- Excellent knowledge of case mangement in the homehealth case management
-Provide CLTC case management services to CLTC participants in person and over the phone during initial, and as needed monthly, quarterly and annual visists
- Conduct assessments to determine clients' needs and develop individualized care plans
- Coordinate and monitor services to ensure clients' needs are met effectively
- Collaborate with internal and external stakeholders to advocate for clients and facilitate access to resources
- Implement crisis intervention strategies when necessary
- Compassionate ith teamwork skills
- Previous experience with psychological aspects of care
- Effective communication skills
- Manage behavioral health issues and provide appropriate interventions
- Demonstrate expertise in crisis management and resolution
- Comply with state ,federal regulations in the delivery of covered services ensuring these are appropriate and that documentation requirements are met in a timely as indicatedby SCDDHHS
**Experience:**
- Bachelor's or Master's degree in Social Work or related field preferred
- Proven experience in case management or related roles
- Strong knowledge of Medicare regulations and guidelines
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Proficiency in crisis intervention techniques and problem-solving skills
