ABOUT HOME FREE
Our Home Free residence is located on Treasure Island (in San Francisco) and is a transitional reentry housing program for criminalized survivors of domestic violence. We house up to 10 formerly incarcerated women who have survived domestic violence, spent long sentences in prison, and are reestablishing their lives. We recognize that the reintegration needs of mature women who are survivors of domestic violence and related trauma are different and thus this is a call to action to be responsive to those survivors for reentry housing and related programs. After long periods of incarceration, the survivors have missed out on cultural and technological changes in society, have been away from family, and have been separated from the workforce for decades. Home Free provides safe and vibrant co-housing that underscores each woman's worth, humanity and dignity for their successful reintegration. This is a place for the women to create pathways to their own independence, success and happiness.

Qualifications
JOB SUMMARY
The Care Coordinator position reports to the Home Free Program Director and is primarily responsible for building trusting relationships with Home Free residents in order to develop and monitor individualized reentry plans and connect residents to essential services that support their independence and success such as obtaining vital documents, connecting to public benefits, attending required therapy/treatment,needed medical care, securing employment. This position will also be responsible for helping coordinate residents' post-program placement to long term housing. The Care Coordinator will work closely with residents' Parole Agents to ensure that residents are in compliance with conditions of parole and are preparing to successfully exit the program between 6-12 months.

You are comfortable working with people from all walks of life, including previously incarcerated individuals and survivors of domestic and intimate partner violence. You will support the Five Keys Home Free mission by creating a supportive environment of mutual respect and fairness, while encouraging creative and critical thinking through an equity lens. In addition, you will contribute to our appreciation of diversity and foster a climate of multicultural understanding.
JOB DUTIES AND RESPONSIBILITIES
Case Management Duties
  • Develop individualized reentry case management plans and connect residents with community and government resources to increase the likelihood of housing stability and ensure compliance with conditions of parole
  • Provide referrals to support services so that residents have a successful reentry experience that includes improving their mental and physical health and increasing their independence
  • Document clear, precise and timely case notes, service referrals, and program progress notes in required client databases per Five Keys and funder requirements
  • Facilitate weekly 1 on 1 case management sessions with residents and monthly case plan reviews with residents and Program Director and/or Assistant Director
  • Participate in case conference meetings with resident, Program Director, Assistant Director, and/or Parole Agents as needed
  • Coordinate services that include job placement assistance, access to adult education, financial and digital literacy, and housing navigation assistance.
  • Support residents with post-program success including connection to housing programs, research of available housing units, support with housing applications, and requesting credit and background reports as needed
  • Help plan and facilitate Life Skills sessions and other needed groups which may include, but are not limited, to topics such as budgeting, household finances, conducting a housing search, nutrition, working with landlords, tenants' rights, healthcare navigation, and trauma-informed programs that promote self efficacy and wellness
  • Maintain protocol and records of client feedback and grievances
  • Support the program expectations, principles and community rules
Administrative Duties
  • Timely input of client data and outcomes into systems required by Five Keys (including Apricot) and state funding agencies, such as ARMS (required by CDCR)
  • Provide monthly and quarterly reports of client activities as instructed by the Program Director of Assistant Director of Home Free
  • Data input may include enrollments, assessments, contacts, units of service, referrals, and case notes
General Duties
  • Provide a welcoming and supportive trauma informed environment for all Home Free residents, colleagues, and program partners and guests
  • Coordinate with CBOs and government organizations to establish connections to and on-site provision of resident resources such as housing, public benefits, employment services, substance abuse treatment, etc.
  • Support with all administrative tasks related to Five Keys Home Free
  • Provide information requested by the Five Keys Administration
  • Maintain accurate records and log all referrals made in Apricot and ARMS
  • Document any programmatic protocol violation accompanied by an incident form
  • Abide by Five Keys and CDCR policies and procedures
  • Provide quality assurance regarding programming and overall case management
  • Other duties as assigned
KNOWLEDGE AND SKILLS
  • Knowledge of best practices in supporting formerly incarcerated individuals
  • Knowledge of trauma informed practices in supporting survivors of intimate partner violence and/or human trafficking
  • The person must be comfortable, mature, and professional in interacting with diverse community partners, participants, and colleagues with diverse heritage and backgrounds
  • A friendly and calm personality with the ability to communicate clearly with participants, colleagues, and program partners
  • Excellent technological skills, proficiency in computer
  • Excellent data entry skills and proficiency in Microsoft Office, databases, intranet, and email, and able to learn and use internal and external database systems
  • Willingness and ability to grow professionally, be accountable, expand skills, and adapt to evolving school environments are required
  • Committed to the mission, vision, and values of Five Keys is necessary, along with an understanding of the challenges and needs of Five Keys participants
  • Ability to maintain strong and appropriate boundaries with personal identifying information when interacting with participants or community or program partners, and while at any site location or community or program partner site
  • Exceptional written communication skills; ability to express complex ideas with clarity and precision in written documents
  • Excellent interpersonal and organizational skills
  • Eagerness to learn new things and adapt to constantly changing circumstances
  • Ability to handle multiple deadlines and tasks simultaneously
  • Advanced problem-solving skills; tactfully address and resolve questions/concerns
  • Proven skills in facilitating groups and providing case management required
  • Proven track record of success in completing administrative tasks preferred
  • Able to maintain strict confidentiality and take all precautions when handling sensitive information
EDUCATION AND WORK EXPERIENCE
  • A minimum of five years experience in providing direct service to a similar population
  • Typically requires 2-3 years of prior case management experience, particularly with the reentry adult population, or an equivalent combination of experience and education
  • Experience with supporting survivors of intimate partner violence and/or human trafficking is preferred
PHYSICAL REQUIREMENTS
  • To promote the health and safety of its employees, those we serve and our local communities, Five Keys requires that all employees must be fully vaccinated against COVID-19 and/or adhere to all weekly testing and safety measures.   • Must wear provided PPE (Personal Protective Equipment) and adhere to all safety guidelines and requirements, when needed

  • Lifting items that weigh up to 25 lbs.; reaching, bending, carrying and stooping within the daily routine of tasks and activities
ADDITIONAL REQUIREMENTS
This position is contingent upon:
  • Submitting proof of clear TB results. Results dated within 30 days prior to hire date are acceptable
  • Clearance of criminal background check required by California Department of Corrections and Rehabilitation (CDCR)
WORK ENVIRONMENT
  • Must be willing and able to work in a transitional housing program on Treasure Island and travel throughout the area to support clients as needed
  • Must be willing to work a combination of shifts that may include day shifts (7am-3pm), swing shifts (3pm-11pm), and weekend shifts (Saturday or Sunday)
  • Willingness to be on call as needed
  • Moderate to high noise level
  • Some personal safety risk working in underserved communities
