FlexStaff is Hiring a _Registered Nurse (Case Manager) - Residential and Habilitation Programs Near Bayside, NY

We are looking for experienced RN's to work for one of our external clients, a community services non-profit organization serving people with developmental disabilities for their residential and habilitation programs, located in the Queens, NY.

This is a permanent, full-time job.

Location: Bayside, NY. Charge Nurse, Mon - Fri, 8a- 4pm, flexible to work as staff nurse when needed, Flexible evenings/ overnights and weekends.

Locations: Little Neck, Marathon Parkway, and Astoria is one assignment as the nurse case manager. 16 client caseload; candidate will travel to the different listed sites. Mon - Fri, 8a- 4pm, Flexible evenings/ overnights and weekends.

In this role you will be responsible for developing plan of care and coordinating the delivery of health care services for persons receiving services in an assigned program unit.

An ideal candidate would be a graduate of an accredited program of professional nursing and have a valid license and current registration as a Registered Nurse in the State of New York.

Responsibilities include, but not limited to:

  • Develops plan of nursing services and oversees the implementation of the health care plan by educating person’s receiving services, DSP/Caregivers and families in the plan of nursing services.
  • Educate, monitor and supervise staff in carrying out ordered treatment plan related to health needs as delegated by the RN.
  • Assigns Direct Care staff to perform health care treatments/procedures after having taken into consideration the complexity of the procedure(s), the skill/experience of the staff and the health condition(s)/status of the person receiving services. Initially trains the staff in the task/activity and regularly observes/monitors to ensure all treatments/procedures are performed correctly.
  • Evaluates the effectiveness of preventive/ongoing health care and makes recommendations/ referrals for alternative care, as necessary.
  • Acts as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providers.
  • Completes and maintain all required written documentation on the approved electronic health record system. Charts health care progress as indicated by acute illnesses and/or complex ongoing medical problems. required
  • Conducts general and individual specific health related trainings for persons receiving services, direct care staff, and other team members as needed.

Job Type: Full-time

Pay: $83,000.00 - $95,000.00 per year
