Job SummaryThe Director of Continuous Quality Improvement (CQI) is responsible for the creating and managing systems that provide meaningful data analysis and feedback loops between the CQI Department and programs that spur programs toward higher quality, maintain compliance, and result in better outcomes for families in Forestdale’s services. As a leading NYC child and family service agency, we have families come to us, often in crisis, seeking help to heal from trauma and gain a path toward future growth and success. This position will work closely with direct reports and program heads to develop and monitor plans for Quality Improvement to achieve regulatory and best practice benchmarks in the service of child and family well-being.
Major Responsibilities
  • Develop and implement on-going quality improvement plan for all agency programs
  • Acts as a liaison for all program evaluation activities that interface with OCFS, the New York City Administration for Children’s Services (ACS), including EQUIP, PAMS, and SCORECARD
  • Review, assess and, when necessary, make recommendations to improve management of documentation for all agency programs
  • Review and create effective agency forms for data tracking/monitoring purposes
  • Develop and monitor outcome measures of performance for program
  • Chair quarterly performance management meetings for each agency program.
  • Develop and review process manuals and workflows for agency staff and programs
  • Oversee the scheduling of Service Plan Reviews/Family Team Conferences
  • Act as Third-Party Reviewer/Facilitator at Service Plan Reviews/Family Team Conferences, when requested
  • Monitor client casework contacts for compliance
  • Provide on-going client and foster/resource parent satisfaction surveys
  • Provide all information necessary for ACS assessment and evaluation systems
  • Implement mechanisms to obtain feedback from clients on service quality and recommended improvements, including a plan for exit interviews with children who are discharged from foster care, birth parents reunited with their children and foster/resource parents to ascertain the effectiveness of Family-to-Family practice and satisfaction with services
  • Responsible for ensuring compliance with all agency policies, procedures and governmental regulations, including but not limited to HIPAA
  • Participate as a member of the agency Senior Staff team to work together finding ways to improve the functioning of the agency and its ability to deliver high quality services that respond to primary community needs
Minimum Requirements
  • Master’s degree in social work, research, data science, or similar field from an accredited college or university
  • A minimum of five years post Master’s experience, including experience in the field of social services, especially child welfare; experience in program monitoring, reviewing, assessing and providing technical assistance
  • An understanding and respect for community needs and cultures
  • Organizational, verbal and written communication skills
  • A concern and commitment to children and families
  • Excellent interpersonal skills
  • The ability to maintain composure and a positive outlook; and to function well under stress
  • Readiness to accommodate a flexible work schedule
  • Availability on rotation as an administrator on duty during off hours;
  • Computer knowledge; experience with CONNECTIONS a plus
