Forsyth County is Hiring a Team Leader Near Cumming, GA

The purpose of this classification is to serve as a lead worker in the meals and food services area of Senior Services. Individuals in this classification are expected to ensure that the tasks pertaining to the area are carried out correctly and efficiently; each team leader works under direct supervision and exercises only limited discretion with regards to team activities.The following duties are normal for this position. The omission of specific statements of the duties does not exclude them from the classification if the work is similar, related, or a logical assignment for this classification. Other duties may be required and assigned.
Coordinates activities of an assigned program/service designed to assist clients of the senior center with various aspects of life: assists the manager with program planning; provides assistance and guidance to team members regarding policies and completion of assigned tasks; serves as a liaison between the manager and team members for the purposes of relaying information and responding to problems or issues; and ensure that all tasks and documentation requirements are completed.
Depending on assignment, Team Lead could lead and participates in the onsite distribution and home delivery of meals for seniors: places daily orders for meals to the contract provider; receives meals from contract provider, serves meals onsite, and boxes meals for home distribution; locates referrals for potential clients to receive homebound meal services; obtains and records directions to client homes; explains program and local guidelines prior to enrollment; completes referral forms, registration forms, and nutritional screening instrument; obtains copies of prescriptions from diabetic clients and places in client files; provides backup coverage for food preparation, meal delivery, client transportation, or other tasks.
Depending on assignment, Team Lead could lead and participates in the Alzheimer’s patient assistance program for seniors: prepares the calendar of program and activities for the day program; provides structured program including activities such as memory stimulation, trivia questions, games, puzzles, crafts, singing, exercise, lunch, and snacks; works to provide clients with a relaxed atmosphere and a sense of security in surroundings and with volunteers; encourages clients to participate in activities; stimulates memory of personal life, birthdays, and happy times; coordinates short expeditions such as town tours, picnics, lake visits, or trips to ice cream store; and carries clients to restrooms as needed.
Depending on assignment, Team Lead could lead and participates in the assessment and registration of new and existing clients: schedules assessments; interviews participants to determine program eligibility, assess level of functioning, and identify interests; registers participants for programs as appropriate; conducts environmental assessments; compiles reports and maintains all related documentation; and coordinates admission and discharge procedures for all program participants.
Depending on assignment, Team Lead could lead and participates in resource development for the program: solicits, trains, and oversees volunteers that will provide program assistance at the senior center; solicits in-kind donations for materials needed; works with manager to identify program needs and potential sources for donations.
Depending on assignment, Team Lead could lead and participates in the compilation and preparation of all reporting and documentation requirements for the senior center and related programs: ensures compliance with all reporting mechanisms required by grants and or state agencies in order to secure funding for the programs; maintains detailed records of all programs and activities; and performs related administrative tasks.
Depending on assignment, Team Lead could lead and participates in the transportation program: coordinates the pick up and drop off of seniors participating in the day program and/or other activities provided by the department; assists with the scheduling of activities requiring transportation and ensures that drivers are available; documents daily trip totals and related activities; schedules maintenance of vehicles; operates a passenger van or small bus to transport clients or other individuals as needed; and ensures that drivers are following proper procedures and guidelines with regards to the transportation program, licensing, and its participants.
Participates in or provides programs to senior center clients: facilitates structured programs including activities such as educational programs, outings, memory stimulation, trivia questions, games, puzzles, arts and crafts, singing, exercise, lunch, and snacks; works to provide clients with a relaxed atmosphere and a sense of security in surroundings and with volunteers; encourages clients to participate in activities and transports clients to and from programs as necessary.
Depending on assignment, Team Lead could interact with and assists program participants as needed, to include activities of daily living: addresses concerns for participants and assists with problem resolution; maintains an awareness of the general well-being of participants; reports any suspicion of problems that might require intervention of Adult Protective Services.
Prepares or completes various forms, reports, correspondence, client reports, waiting lists, referral forms, registration forms, nutritional screening instrument forms, calendars, mileage reports, reimbursement reports, or other documents.
Receives various forms, reports, correspondence, referrals, maps, medical resources, product instructions, policies, procedures, manuals, directories, reference materials, or other documentation; reviews, completes, processes, forwards or retains as appropriate.
Operates a computer to enter, retrieve, review or modify data; verifies accuracy of entered data and makes corrections; utilizes word processing or other computer programs.
Communicates with supervisor, employees, students, volunteers, other departments, clients, passengers, the public, community agencies, outside agencies, and other individuals as needed to coordinate work activities, review status of work, exchange information, or resolve problems.
Provides articles or information for the center newsletter, webpage, and other communications.
ADDITIONAL FUNCTIONS
This classification is designated as a safety sensitive position which is subject to random drug and alcohol testing and any other drug testing methods as stated in policies adopted by Forsyth County concerning drug and alcohol.
Performs general/clerical tasks, which may include answering telephone calls, making copies, sending/receiving faxes, filing documentation, picking up or delivering mail, or conducting visitor tours of senior center.
Performs other related duties as required.High school diploma or GED; supplemented by one (1) year previous experience providing programs and services to senior, with supplemental training in the area of assignment; or any equivalent combination of education, training, and experience which provides the requisite knowledge, skills, and abilities for this job. Must obtain ServSafe Food Safety Manager Certification within 90 days of beginning of employment. Must possess and maintain valid CPR/First Aid certification. Some positions require a valid Georgia driver’s license.
