Position Overview
Program Coordinator – Health and Well-being
Salary: $44,000
Accepting Applications until June 21, 2024. Review period will begin June 24, 2024.
As part of the Foster Success team, the Coordinator - Health & Well-being, advances the organization’s mission by ensuring teens and young adults have access to a continuum of Health & Well-Being programs and resources as they transition out of foster care. The coordinator serves as the point of contact for young people seeking support & resource connections and works with the Director of Health & Financial Well-being to ensure the smooth functioning of program operations and activities including the Basic Needs program, the Mental health Acceptance, Promotion, and Support Initiative (M.A.P.S.), Vital Documents, “Real Adulting” workshops, and the Insuring Foster Youth Trust Fund. This position is responsible for connecting participants to a spectrum of resources, knowledge, and skills that enhance their overall well-being. The Coordinator- Health & Well-being will support in cultivating a culture of well-being across Foster Success’ impact areas among our mission population.
The Health and Well-being Coordinator reports to the Director of Health & Financial Well-being.
This position will begin full-time, in-office (based in Indianapolis), and will have an opportunity to become a hybrid or fully remote position.
Organization Overview
Foster Success mission is to Foster Success’s mission is to hear, support, and empower teens and young adults transitioning out of foster care on their journey to self-sufficiency. We are committed to improving the foster care system and ensuring that teens and young adults transitioning out of foster care are educated, housed, financially stable, employed, and connected to a support system by age 26. Our team accomplishes this through program delivery, policy change, and partnership developments in the areas of K-12 & post-secondary education, workforce development, health & well-being, financial empowerment, housing stability, and youth engagement.
Primary Responsibilities
  • Program Administration:
    • Coordinates the day-to-day activities of Health & Well-being programs, specifically the Basic Needs program and M.A.P.S. Initiative. Review applications, collect required documentation, complete intake assessments, and disburse funds and referrals in a timely and organized manner. Serve as the point of contact for all young people in the Basic Needs program and M.A.P.S. Initiative. Provide input on system and process improvement. Problem solve and troubleshoot challenges as they arise. Support in additional, occasional projects as needed.
  • Participant Support
  • Complete Basic Needs and M.A.P.S. Intake processes and serve as a point of contact for young people seeking support through 1:1 referrals and connections to community resources in areas including but not limited to: Housing, Financial Support, Medical & Health Needs, Self-Advocacy, and Basic Needs (i.e. food, hygiene items, etc.). Empower young people in building long-term support networks. Engage in skill-building and relationship building work as necessary to fully support each young person in their well-being needs. Plan and coordinate quarterly “Real Adulting” workshops on various topics, in collaboration with colleagues, community partners, and young people.
  • Payments
    • Process payments for Basic Needs, Vital Documents, and Insuring Foster Youth Trust Fund requests. Work with third-party vendors to ensure timely payments are made. Track day-to-day spending to ensure compliance with monthly budgets. Track receipts and ensure organized record keeping.
    • Data Tracking and Reporting
      • Update documentation in the Salesforce system on a daily basis. Provide support in the management of participant databases and keeping up-to-date information. Support in data collection and analysis.
    Position Requirements
    • Bachelor’s degree with 2 years of experience working with individuals affected by foster care, individuals from diverse backgrounds, or involved in the justice system
    • Demonstrated success in the areas of program implementation, service coordination, and attention to detail, Experience working with transition aged youth between the ages of 14-26 from diverse backgrounds Experience facilitating learning and activities for youth or young adults
    • Mental health knowledge/experience is preferred,
    • Ability to represent Foster Success with senior-level partners, agencies, and elected officials.

    • Work a flexible schedule that includes some weekends and evenings to meet program goals.
    • Hold a valid Driver’s License and have access to a vehicle or the ability to rent a vehicle for work-related travel.
    • Demonstrate proficiency in computer skills and experience with and knowledge of relevant software, particularly Salesforce, Zoom, Google, and Microsoft Products.
