Fostering Life-Changing Opportunities is Hiring a Remote Community Support Specialist

The Flourish Community Support Specialist (CSS) provides case management and service coordination to youth enrolled in Flourish’s programs and to their caregivers when it may support the overall wellbeing goals of the youth. Case management and service coordination is a dynamic process that assesses youths’ needs, works with youth to create and implement plans, and then evaluates the plan’s activities towards improving participant outcomes or self-selected goals. This position is for someone who is passionate about enhancing lives and who will work to deliver results with integrity to positively impact youth, team members, and our community partners. Job duties may include, but are not limited to: Engage and build trust with youth to establish strong rapport. Assess youths’ needs, risk factors, barriers, and strengths through surveys, interviews, and check-in meetings. Assist youth with goal-setting in all areas of well-being and help them develop the skills needed to achieve those goals. Collect and manage youth information required to participate in Flourish programs/services and maintain confidentiality of all youth Personal Health and Personal Identifying Information (PHI, PII) Identify and address basic needs effectively when crises may arise. Help youth and families navigate community resource systems (SNAP, Medicaid), advocate, and provide application assistance to improve access. Assist youth in applying for employment and navigating education programs. Document, track, and provide reports on case management activities and progress. Build and maintain positive relationships with youth, families, schools, nonprofits, and business partners to strengthen the network of resources and positive relationships. Support special projects involving Flourish youth. Arrange transportation for youth and complete other daily tasks/errands that support youths’ progress towards meeting their goals. Stay up to date on evidence-based best practices and the landscape of community resources and continuously incorporate that knowledge into practice. Monitor and evaluate progress with youth and strategically adjust approaches when necessary. Adhere to Flourish’s clinical case management model, best practices, and administrative requirements with support from Program Coordinators and other Specialists, and through supervision provided by the Community Program Manager. Other duties as assigned. We’ll also want to know you can demonstrate these attributes: Effective communication and conflict resolution Ability to manage time and prioritize to meet deadlines Thrive in a highly cooperative work setting Organizational and information management skills High attention to detail Exemplary customer service skills and client-centered approach Excel in a fast-paced, results-oriented environment Ability authentically engage and build trust with diverse individuals Bachelor’s Degree or higher in relevant area of study (Social Work, Education, Sociology, Human Development, Public Health, etc.) and/or experience working with youth in public education or other community human service organizations Valid Missouri driver's license Must be able to pass state and federal background screening Experience working with marginalized youth and/or families Skilled in public speaking and creating presentations and/or trainings Project management and/or program coordination experience Proficient with Microsoft Outlook, Word and Excel, and other basic computer technologies Veterans United Home Loans and its affiliates are proud to be Equal Opportunity Employers committed to creating a diverse and inclusive workforce.
