Essential Duties & Responsibilities
Responsible for performing initial and ongoing psychosocial assessments of patients/family/caregiver with a holistic perspective that incorporates identification of support systems, coping mechanisms and barriers to care (for example healthcare literacy, safety abuse and neglect, social determinants of health, suicide risk, spiritual well-being, patient capacity etc.)
Utilize therapeutic techniques to help patients and families understand options and participate in healthcare decision-making to include goal setting.
Provide emotional support, counseling, complex case management services to high acuity PC patients and families (high acuity may be defined as patients or families that are already involved with mental health providers, SI/HI, substance abuse, etc).
Facilitate care coordination based on identified needs. Serve as liaison to community health, welfare, and social agencies and make referrals to appropriate agencies on behalf of the patient and family.
Assist with patient and family education with regards to health literacy, cognitive abilities, communication patterns, challenges, and learning styles. (Examples of areas for education could include: serious illness trajectory, Palliative or Hospice Benefits, self-care, reinforcement of care plan, advanced care planning, non-pharmacological symptom management)
Engage patients, hospitals, facilities, and community in care and education of Palliative Care services
Ensure continuity of care across service lines and settings to include advocacy for patient centered care
Actively participate in IDG team meetings for development and evaluation of the patient’s plan of care
Facilitate end of life conversations including completion of advance healthcare directives
Assessment and support for patient and family related to anticipatory grief with regards to family systems/dynamics and coping methods. Evaluate need for bereavement counseling or appropriate interventions related to complicated grief.
Assess current and desired quality of life to facilitate appropriate and measurable goals
Provide Comprehensive Clinical Assessments to patients referred for brief psychotherapy intervention (PC LCSW Scope of practice includes anxiety, depression, anticipatory/complicated grief support and adjustment disorders) Patients with severe and persistent mental illnesses will be referred to a mental health provider for more comprehensive services

Knowledge of HIPAA privacy rules and regulations
Knowledge of electronic medical records systems
Expertise in discharge planning
Knowledge of Hospice Services to include HMB
Demonstrate basic data entry skills

Minimum Qualifications Education:
Master’s in Social Work (MSW)
Experience
1-3 years experience within a healthcare setting
Licensures & Certifications
Valid NC, Clinical Social Worker license (LCSW)
Valid driver’s license and proof of automobile insurance
Hand dexterity to include gripping, pushing/pulling and writing
Other Duties
Work to achieve performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values and competencies
Perform duties in compliance with Medicare Condition of Participation
Participate in the coordination of the patient’s hospice care in accordance with professional standards of practice
Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation
Develop and maintain professional relationships with all members of other teams involved in client’s care
Demonstrate the ability to function in a variety of care environments, including home settings, skilled nursing facilities, assisted living environment, etc.
Evaluate bereavement/pre-bereavement needs of client/family/caregiver and develop bereavement plan of care based on that evaluation
Responsible for obtaining the necessary information from third party providers for payment
Responsible for facilitating internal referrals
Assist in providing after hours services if necessary.
Conduct pre-admission consults and/or facilitate completion of consent forms
Attend staff and mandatory in-services meetings, as required
Participate in ongoing education and assume responsibility for updating professional skills
