FOXDALE VILLAGE is Hiring a Medical Home RN Supervisor (FT 80 hours; M-F, 8:30am-5pm) Near State College, PA

DescriptionFoxdale Village is a Quaker-directed, non-profit Continuing Care Retirement Community (CCRC) located in State College, PA. Foxdale Village was founded on the values of treating every person with dignity and respect and actively strives to create an engaging and welcoming environment for all. We are seeking a full-time Medical Home RN Supervisor to join our caring and dedicated team. This RN position works M-F 8:30am-5pm, 80 hours per two-week pay period; 5, 8.5-hour shifts per week.Medical Home RN Supervisor Job Duties Include
  • Foster a collaborative approach in working relationships with other Health Services staff as well as residents, their families and friends and other departments in order to ensure smooth provision of health services, health promotion, and continuity of care.
  • As one function of the Medical Home serves as a home base or gateway for medical needs, assist residents to arrange for appointments with providers external to Foxdale, as well as ancillary services including pharmacy, lab, and medical tests).
  • Participate in the evaluation of residents' health status, development of care needs, and evaluation of treatment plan.
  • Participate in the evaluation and prioritization of resident requests to see the physician.
  • Assist physician in the coordination of the day-to-day operations of the Medical Home.
  • Administer TB skin tests and Hep B vaccines to employees.
  • Supervise, mentor, train and orient other Registered and Licensed practical nurses in the medical home. Instruct as needed in order to ensure safe and competent care.
  • Complete and administer individual development plans within established deadlines and according to Foxdale’s performance management program guidelines.
  • Participate in the development of procedures and standards of care relative to resident care and use of medical equipment in the Medical Home and independent living residences.
  • Maintain home health medication management systems and follow policies of storage and administration of all medications.
  • Demonstrate competence in executing physicians’ orders and instructing residents regarding medications, treatments, diagnostic tests and preventive health. Follow-up of physician orders is timely and thorough.
  • Provide first aid for employees and residents as appropriate.
  • Provide or identify sources of home health care to apartment residents following established policy.
  • Administer medications as prescribed by the physician following policies and procedures. The RN may administer I.V. medications if he or she has been certified to do so, following Foxdale Village’s I.V. policies.
  • Provide follow-up services with apartment residents after discharge from acute care, Anthony House or Darlington House.
  • Demonstrate clinical competence in responding to emergency calls, the usage of specialized equipment, and implementation of nursing care procedures.
  • Work with the Residency Planning staff to facilitate the pre-admission process for new residents in a timely manner.
  • Maintain inventory of medical supplies, equipment, forms, and sterile instruments.
  • Assume responsibility for completing and maintaining appropriate records and documentation in a timely and legible manner.
  • Maintain security of medical records and confidentiality.
  • Assume responsibility for and maintain podiatry schedule for all Foxdale Village residents.
  • Participate in the auditing of apartment resident charts for accuracy and completeness.
  • Follow established nursing, pharmacy, and infection control policies, procedures, and standards of care.
  • Participate in the RN supervisor on call schedule
  • Participate in monitoring and evaluation activities for quality assurance.
  • Uphold the rights of residents. Communicate effectively through written and non-written methods to assure continuity of care and efficiency in work responsibilities.
  • Participate regularly in Health Services supervisory meetings and as member on assigned committees, such as Risk Management and Compliance committees.
  • Participate in staff development programs and encourage opportunities for growth and individual development for staff and residents..
  • Maintain punctuality and regular and predictable attendance in order to meet the daily medical needs of the residents

RequirementsMinimum Qualifications Include:
  • Graduate of an approved National League of Nursing (NLN) school.
  • Licensed to practice nursing as a Registered Nurse (RN) in the Commonwealth of Pennsylvania.
  • Possess a valid CPR card.
  • Minimum of one (1) year experience as a professional RN is preferred.
  • Experience in the field of gerontological nursing is desired.
