FPHNY Recruitment Team is Hiring a 10101 - Community Health Worker Near New York, NY

Community Health Worker The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers. To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City. PROGRAM OVERVIEWThis position will be based within the Bureau of Health Promotion of Justice Impacted Populations (BHPJIP) in the DOHMH Division of Mental Hygiene. Through the use of data, policy, and program implementation, BHPJIP promotes community-based, health-led, cross-sector strategies that reduce criminal-justice involvement and improve the transition of individuals back to the community after incarceration and community supervision. BHPJIP seeks to improve the physical and behavioral health and well-being of marginalized populations, with specific emphasis on persons involved and impacted by the justice system, by focusing on the role that healthcare and public health can play in addressing their complex health and social needs. POSITION OVERVIEWThis position sits within the New York City Health Justice Network (NYC HJN), an innovative program that aims to holistically improve the physical, behavioral and emotional health and wellbeing of persons with justice involvement (PWJI) who are returning from incarceration. PWJI are defined as anyone who has had direct contact with any level of the criminal justice system (i.e., incarceration, probation, parole, police contact, etc.) Utilizing trained Community Health Workers with lived experience of the justice system, the program aims to increases clients access to, engagement in, and quality of community-based healthcare and social services. The NYC HJN connects primary care and community-based reentry organizations (CBOs) with experience serving PWJI to serve NYC HJN clients, employing an innovative client engagement strategy that includes: 1) employing community health workers with lived experience of the criminal justice system working as navigators between the health and social systems; and 2) implementing trauma informed care interventions in clinical and community settings. RESPONSIBILITIESMaintain strong relational presence within identified primary care clinic(s) and CBOsConduct community outreach and provide education, on primary care and social services, to persons impacted by the justice systemMaintain a caseload of client participants including goal setting, creation of a participant action plan, and reinforcement sessionsAccompany participants to medical and social services visits, as neededMaintain up-to-date documentation and data entry in accordance with organizational guidelinesPrepare case-related reports including outcomes, successes and challengesEngage in routine case conferencing with CHW Supervisor and clinical and social service teamsComplete required in-house trainings including, CHW training and data platform training, and other trainings as required by the primary care clinics and community-based organizations Perform other duties as assigned QUALIFICATIONSCandidates must have with lived experience of the criminal legal systemHigh school diploma or GEDBilingual preferred (English/Spanish)Experience providing coaching, mentoring, adult learning, counseling, outreach, workshop facilitation, and/or group/community presentations preferredExperience working with marginalized populations, particularly PWJI, preferredA commitment to collective action and community organizing preferredAbility to work collaboratively and in a fast-paced environment SALARYSalary range is $47000 to $ 57200 BENEFITS AT GLANCEFPHNYC offers a comprehensive benefits package:Public Service Loan Forgiveness (PSLF) eligible employerGenerous Paid Time Off (PTO) policyMedical, dental, and life insurance with low or no employee contributionA retirement savings plan with generous employer contributionFlexible spending medical and commuter benefits planMeaningful work at an organization striving to advance health equity and social justice ADDITIONAL INFORMATIONFull Time M-F 9-5 hybridLocation: Gotham Department of Health and Mental Health 42-09 28th St., Queens, NY 11101 All new hires must be vaccinated against the COVID-19 virus unless they have been granted a reasonable accommodation for religion or disability.
