Franklin Primary Health Center is Hiring a SCHOOL BASED CLINIC PROGRAM MANAGER Near Mobile, AL

Job Summary:

The School Based Clinic Program Manager will lead the implementation of full-service community school-based health clinics across the organization’s service area. This includes but is not limited to the Mobile Area Education Foundation (MAEF) Public Charter School and the Chickasaw City Schools, in partnership with local school district nurses. This position is responsible for overseeing the day-to-day operations of the school-based health clinics, ensuring high-quality student-centered care and adherence to regulatory standards. The school based clinic program manager will work in collaboration with the school nursing staff ensuring that they receive orders/directions from the physician/nurse practitioner providing clinical services. The School Based Clinic Program Manager will assist in development and administration of all school-based contracts, ensure compliance with grant/contract requirements and ensure that all requirements including reporting is completed and submitted in a timely manner. This position will develop policies and procedures, work collaboratively with school administration and healthcare providers to provide behavioral health and social service support and promote student and community health and wellness. Ensures Franklin’s compliance to all school required policies including FERPA.

DUTIES AND RESPONSIBILITIES:

  • Develops and implements long and short-range goals and objectives for Community School Based Health clinics.
  • Works collaboratively with MAEF Public Charter Schools and the Chickasaw City School health clinics and local school district nurses.
  • Oversee daily operations of the clinic, including student flow, staff scheduling, and inventory management.
  • Responsible for the planning, development, implementation and evaluation of specialized initiatives designed to address problems influencing individual and family functioning. Specialized programs will focus on chronic disease as well as preventative wellness initiatives in all life cycles.
  • Community Liaison. Provide linkages to organizational and community resources as necessary.
  • Participates with other members of the medical team in the development, implementation and evaluation of the school-based programs.
  • Coordinates case management activities to support programs and services of the Center and cooperates in educational and training activities.
  • Develops/implements and evaluates monthly and periodic initiatives to increase the number of users as well as the number of encounters at the school-based locations.
  • Oversees the development of individualized case management plans as required for chronic care patients.
  • Assists with in-service programs, community presentations, health education activities including coordination and participation in health fairs and other outreach activities.
  • Responsible for overseeing the completion of reports required under specialized program initiatives.
  • Stays abreast of changes in Medicaid and other third party payer programs in order to assist patients/families with efficiently utilizing resources.
  • Other duties as assigned

LICSW Duties:

1. Responsible for completing Mental Health/Substance Abuse Assessments/screening and provides counseling utilizing evidence based interventions.

2. Provides individual, group and/or family counseling to patients and families experiencing behavioral health issues.

3. Provides treatment options, bereavement counseling, crisis intervention and emergency services.

4. Provides case management for patients, when appropriate.

5. Facilitates and refers patients/families to appropriate levels of care when necessary

6. Establishes and maintains clinical charts, referrals, and other related documents for the treatment of FPHC patients; prepares related documentation; keeps abreast of patient data to ensure appropriate treatment and care is delivered.

7. Works in collaboration with other program staff, and local community based organizations, to coordinate services in order to avoid duplication of services.

8. Attends professional meetings, educational conferences, and in-service trainings in order to maintain and improve professional competence.

QUALIFICATIONS:

EDUCATION AND EXPERIENCE: Masters of Social Work or Master degree in counseling, Behavioral sciences, or other Health related Field with two or more years” work experience; Possess license in the State of Alabama. Possess strong administrative, management and supervisory experience.

KNOWLEDGE, SKILLS AND ABILITIES:

Demonstrated knowledge of case management practices and skills, program development, referral sources, mental health diagnosis and treatment, alcoholism and chemical dependency. Skills in establishing and maintaining effective working relationships with patients, families, medical staff, and the public are required. Skills are required in preparing and maintaining records, writing reports, and responding to correspondence. Must possess the ability to communicate clearly and maintain confidentiality. Ability to prioritize and organize caseload. Possess the ability to instruct, delegate and supervise a diverse team. Basic office skills such as data entry, filing, phone etiquette, and composition of business letters, and professional proposals. Must possess strong communication skills; verbal, oral and written. Possess outgoing and friendly personality with ability to organize time efficiently among a variety of duties. Ability to present ideas to community, professional groups, establishes effective working relationships and the flexibility to undertake a variety of assignments. Must be able to express ideas clearly and concisely; and exercise balanced judgment in evaluating situations and making decisions.

CERTIFICATIONS, LICENSURES AND REGISTRATIONS:

State of Alabama Social Worker's license, valid driver’s license, auto liability insurance.

Job Type: Full-time

Pay: $75,000.00 - $90,000.00 per year
